Works for the Central Virginia VA Health Care System (CVHCS) in Richmond, Virginia, as a direct report to the Social Work Service, and is assigned to the Community Adult Day Healthcare (ADHC) Program, VA and Community PACE Program and primary COR for Ensocare Contract within Geriatrics & Extended Care (G&EC).
Duties
Responsible for overall development, management, operations, policy development and evaluation of these programs; ensuring compliance with all applicable program requirements.
Duties include: reviewing and reporting data and statistics, monitoring, analyzing, planning, policy creation/development, and education to facility and community staff. The incumbent is also responsible for reporting of adverse/sentinel events per facility policy. Additional duties include serving as primary coverage for the Community Nursing Home (CNH) Program Social Work Coordinator and work in other NIC Programs as assigned by the NIC Social Work Supervisor.
Serves as the primary contact for the C-ADHC Program and is actively involved in management of consults to the program, collaborating with referring teams as appropriate, communicating with Veteran and caregivers in regards to the C-ADHC Program, ensuring satisfactions surveys are completed per program requirements, and providing education about the C-ADHC program to staff, Veterans, and community.
As the VA and Community PACE Program Coordinator the incumbent will develop and fortify program operational development, foster and continue to build relationships with PACE and is responsible for education to staff, Veterans and the community, serving as the liaison between PACE and VA, responding to consults and reporting data and statistics to facility leadership as requested.
The incumbent will serve as the primary Contract Officer Representative for Ensocare contract.
Will provide primary coverage to the CNH Program Coordinator. The incumbent maintains level II certification as a Contract Officer Representative (COR). If the incumbent is not currently certified at a level II COR they will work to achieve this certification within 2 years and will serve as the alternate COR on all CNH Program Contracts.
Work Schedule: Monday-Friday, 8am - 4:3-pm
Telework: Ad Hoc - as determined by the agency policy.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Policy.
Education. A master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. GS-12 Social Worker
Experience and Education- One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification- Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities- In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.

This position is eligible for
the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
