The Chalmers P. Wylie VA ACC is seeking for a Licensed Practical Nurse for Newark Community Based Outpatient Clinic. The TCT LPN/LVN that serves in a generalist role to support Telehealth clinical programs from the patient location. S/he provides a wide range of clinical and technical services to the Veteran that demonstrates complex technical judgment under general supervision of a Registered Nurse (RN) or MD.
Duties
The LPN demonstrates nursing practice with remote supervision of an RN or physician.
Administers medication accurately and promptly under the general supervision of an RN. Demonstrates understanding of the effects and/or reactions caused by medications, treatments, and procedures.
The LPN demonstrates independent technical knowledge and skills that are complex in nature and that contribute to the Veteran's treatment plan and support the successful completion of the Telehealth visit.
The LPN is responsible for the patient site facilitation of the clinical, business, scheduling and technical aspects for real-time telehealth applications, including, but not limited to, tele-mental health, primary care tele-health, tele-rehabilitation and tele-patient education clinics.
Uses video-conferencing technologies, telehealth technology and scheduling software to coordinate and connect staff, resources, patients and providers in the manner effective to delivery of services, patient care, education and training.
The TCT assists in patient care under the direct supervision of an independent licensed healthcare provider. S/he can be cross-trained to perform a range of direct care duties that may include, collecting vital signs and other clinical components of a primary care visit as directed by policy or clinic standard.
Identifies subtle changes in patient condition, both physical and emotional. Demonstrates practical knowledge of human behavior, motivation and reactions to situations. Reports these in a timely manner and initiates appropriate action.
Completes required patient care documentation and records significant observations and actions utilizing the nursing process in a clear and concise manner.
Displays a helpful, proactive attitude toward customers by answering questions and resolving problems to enhance customer satisfaction.
Consistently practices customer service performance expectations as established by CARE (connect, appreciate, respond, empower) model.
Accepts assignments willingly, volunteers to help others, and works cooperatively and effectively with co-workers and supervisors displaying a helpful, positive attitude.
Work Schedule: Monday through Friday; 8:00 a.m. to 4:30 p.m. Schedule may vary as deemed necessary to meet the needs of the Veterans and the Medical Center.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Education - Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment OR granted a license by a jurisdiction that does not require graduation from an approved school AND have at least one year of successful practice as an LPN/ LVN.
Licensure - Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico), of the United States or the District of Columbia.
Grade Determination: In addition to the basic requirements stated above, the following qualification criteria must be met. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
GS-03: Experience and Education. None beyond the basic requirements.
GS-04: Experience. Six months of qualifying experience as an LPN or LVN.
OR
Experience and Education. Graduation from an approved school and one year of experience that involved nursing care work in a hospital outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures.
OR
Education. Graduation from an approved school of at least 24 months duration.
LPNs at the GS-4 grade level perform all duties expected of employees at the GS-3 grade level; however, they are expected to exercise greater judgment, require less supervision, and to operate in accordance with all established policies, procedure, and techniques.
GS-05: You may qualify based on your experience or education as described below:
Experience. Completion of at least one year of qualifying experience at the GS-4 level or equivalent and fully meeting all performance requirements for the GS-4 LPN/LVN. Physical Requirements: This position requires potentially long periods of continued walking, standing, and sitting. The Telehealth Clinical Technician/LPN may be exposed to infected patients and contaminated materials and may be required to don protective clothing. S/he may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. S/he must be a mature, flexible, sensible individual capable of working effectively in stressful situations, and able to shift priorities based on patient needs. Applicants must possess the mental and physical ability to satisfactorily perform the duties of the proposed assignment. Physical requirements may involve lifting, carrying, pulling, pushing, reaching above shoulder, walking, standing, sitting, kneeling, and repeated bending. S/he must complete annual Employee Health requirements, such as TB screening or testing on hire, as a condition of employment. In the performance of duties, s/he may be required to drive and/or ride in GSA-vehicles.
