The Western New York VA Medical Center is currently seeking a Registered Nurse - Care Management and Transition Team Coordinator. This position is responsible to lead organizational change and transformation through positioning and implementing innovative learning strategies with emphasis on leadership skills related to Care Coordination, Quality and Patient Flow Competencies of: Leading change, improving patient care delivery, Aligning Resources and Driving Results.

Duties

The Care Management and Transition Team Coordinator (Registered Nurse) has primary responsibility for planning, developing, implementing, directing, and evaluating the overall effectiveness of the Care and Case Management Program across the medical center's inpatient bed programs and transitions to the most appropriate level of care. The Care Management and Transition Team Coordinator develops and maintains effective working relationships with key leaders, VISN staff, VHA headquarters program officials. The incumbent is responsible to the Executive Director and works collaboratively with medical center leadership team and nurse managers when addressing patient flow and care coordination issues. The incumbent will direct activities in the development and achievement of organizational goals to improve Length of Stay, Care Transitions, Quality Metrics, Patient Satisfaction. Responsibilities include develop a comprehensive care management team that takes a more proactive approach to patient transitions and improved documentation of care and EPRP quality measures and manage utilization and transitions to home or community placement, capturing all the critical data points for quality measures and patient/family communications reflected in patient experience measures. These responsibilities will include bed management coordination with authority over patient movement and is a critical part of the transition team. This position will help initiate swift admissions and transfers and serve as an inventory management specialist assuring bed availability and placement at the correct level of care at the right time and place to improve internal movement of patients to reduce bottlenecking in the ED and delaying discharges from acute to home and alternative levels of care in the community. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: At least 3 years of Care and Case Management Demonstrated ability to collaborate with professional disciplines in assuring standards of care and practice are maintained, as well as the ability to develop, implement and analyze performance improvement activities Demonstrated ability to develop, implement, evaluate and interpret professional nursing goals, medical center policies, procedures, standards of nursing and assist the nursing staff in the implementation of the same Demonstrated leadership in developing productive working relationships with groups in other programs, services, academic settings and community agencies, to design, modify and implement systems compatible with professional standards, the mission and the goals of the organization to improve the cost effective use of resource Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Nurse IV - Master's degree in nursing or related field with BSN and approximately 4-5 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and appropriate basic or advanced certification desired, and approximately 3-4 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: This position requires potentially long periods of continued walking, standing and sitting. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing. The incumbent may be exposed to patients who are combative secondary to delirium, dementia or psychiatric disorders. The incumbent must be a mature, flexible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. May need to cover for nurse managers in their absence or during vacancies.
