The Home Telehealth (HT) Registered Nurse (RN) Case Manager performs the primary functions of assessment, planning, facilitation, advocacy, monitoring and evaluation which are achieved through collaboration with the Veteran and other health care professionals involved in the Veteran's care. The HT RN Case Manager exhibits specialized skills and knowledge throughout the case management process and while caring for Veterans and their support systems.

Duties

The Home Telehealth (HT) Registered Nurse (RN) Case Manager performs the primary functions of assessment, planning, facilitation, advocacy, monitoring and evaluation which are achieved through collaboration with the Veteran and other health care professionals involved in the Veteran's care. The HT RN Case Manager exhibits specialized skills and knowledge throughout the case management process and while caring for Veterans and their support systems. The essential knowledge areas for the HT RN Case Manager include, but are not limited to: funding sources, health care services, human behavior dynamics, health care delivery and financing systems, and clinical standards and outcomes. The essential skills for the HT RN Case Manager include, but are not limited to: positive relationship building, effective written and verbal communication, effective collaboration strategies, ability to effect chance, ongoing evaluation and critical analysis, and ability to plan, organize, and execute effectively. Duties include, but not limited to:- Conducting a comprehensive assessment of the Veteran's health and psychosocial needs, including health literacy status and deficits.- Development of case management plans of care collaboratively with the Veteran and family or caregiver. - Planning with the Veteran, family and caregiver, the primary care physician or provider, other health care providers, and the community, to maximize health care responses, quality, and cost-effective outcomes. - Facilitating communication and coordination among members of the health care team, involving the Veteran in the decision-making process in order to minimize fragmentation in the services and maximize efficiency and cost-effectiveness. - Educating the Veteran, the family or caregiver, and members of the health care delivery team about treatment options, community resources, private insurance and service connection benefits, psychosocial concerns, and case management services, so that timely and informed decisions can be made. - Empowering the Veteran to problem-solve by exploring options of care, when available, and alternative plans, when necessary, to achieve desired outcomes. - Encouraging the appropriate use of health care services and striving to improve quality of care and maintain cost -effectiveness on a case-by-case basis. - Assisting the Veteran in the safe transitioning of care to the next most appropriate level. - Striving to promote Veteran self-advocacy, self-determination, and right to choose including refusal of care. - Implementing innovative technological modalities of care provision to sustain collaboration between the Veteran and the health care team thereby accelerating Veteran education and self-management. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Evidence of Belt/Systems Redesign training/experience/certification SNT/GNT/RNTTP completion
