The duties and responsibilities are carried throughout the HealthCare Center including all clinical and other patient care areas involved with the services.

Duties

Duties may include, but are not limited to: The HUD/VASH Housing Specialist provides direct clinical and case management services to Veterans enrolled in the HUD/VASH program. The incumbent will provide consultative and collaborative services to other Health Care for Homeless Veterans (HCHV) programs, including offering clinical services to Veterans enrolled in other HCHV programs as needed . The HUD/VASH Housing Specialist will be responsible for maintaining a caseload of HUD/VASH enrolled Veterans. The determination of the size of the caseload will be dependent upon the complexity of the Veterans on the incumbent's caseload and the needs of the HCHV and HUD/VASH programs. The social worker will develop or contribute to the development of a psychosocial assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others. The goal of the assessment is to highlight the Veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the Veteran's functional status. The incumbent demonstrates an understanding of the range of treatment and skills needed for all adult patients. It is the responsibility of the incumbent, working within the interdisciplinary treatment team, to develop a recovery plan with the Veteran based on this assessment and the Veteran . Working with the interdisciplinary team the social worker is responsible for assisting the Veteran to develop a recovery plan, and encouraging achievable treatment goals with the assigned Veteran and his/her family. The incumbent will refer and encourage Veterans, as appropriate, to utilize community based agencies and resources and other recovery programs to assist the Veterans meet their recovery goals. The HUD/VASH Housing Specialist functions as an ongoing housing specialist on the HUD/VASH Program's team for Veterans who need psychosocial support to facilitate independent community living by coordinating contact between the HUD/VASH program staff and landlords/property management companies and realtors, Public Housing Authorities (PHAs), HUD and any entity connected to helping HUD/VASH Veterans to obtain and retain permanent HUD/VASH subsidized housing. As appropriate completes release of information, and assists with completion, submission and follow-up regarding VA eligibility (DD214) and enrollment (10-10EZ) and other VA Forms or application for benefits through VBA. The incumbent serves as a consultant on the complex processes Veterans encounter when interacting with government and community agencies. The incumbent works staff to ensure referral and linkage to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs connected to housing placement and retention. If appropriate, the incumbent provides referrals or consults to other VA housing programs. The incumbent collaborates with HUD/VASH participating PHAs, HUD, VA and non-VA homelessness/housing search programs and agencies as appropriate to develop viable, individualized housing search plans. The incumbent participates as an active member of the HUD/VASH treatment/case management team, providing and coordinating housing services in concert with clinical treatment using evidence-based models appropriate to the homeless and chronically homeless Veteran population, including, for example, Housing First, Motivational Interviewing, Critical Time Intervention, Harm Reduction or other evidence based interventions. The incumbent maintains working relationships with a network of referral resources. The incumbent assists Veterans in maintaining appointments at public housing authorities, VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services consistent with Veteran's goals associated with healthful independent living. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-11 Social Worker candidates must have the following: Experience and Licensure: Appointment to the GS-11 grade level requires completion of a minimum of one (1) year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR Education: In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one (1) year of professional social work experience in a clinical setting. These are the KSA's for this subject position: GS-11 Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive/operate government vehicle.
