The Primary Care Provider will provide primary care, to include Women's health care, for a panel of approximately 1200 patients at the Oak Lawn Community Based Outpatient Clinic.
Duties

A. General Description:
The Primary Care Provider will provide primary care, to include Women's health care, for a panel of approximately 1200 patients at the Oak Lawn Community Based Outpatient Clinic (CBOC), one of six CBOCs for Hines VA hospital (others located in Hoffman Estates, North Aurora, LaSalle, and Joliet). Incumbent works collaboratively with local CBOC staff and projects involving CBOC staff or patients. Incumbent will participate and maintain clinical and educational activities within Primary Care/General Medicine at Hines to stay grounded and relevant in the role. Women's Health Care updated training for VA care delivery will be provided as needed. Must be Board Certified or Board Eligible in Internal Medicine. Responsible for all functions which fall under the scope of care of Primary Care.
B. Functions: Responsible for all functions which fall under the scope of care of a Primary Care Provider. These include predominantly clinical and administrative management, either directly or through delegated authority of the following:
- Initial patient evaluation
- Management of acute and chronic medical conditions in an outpatient setting:
- Follow up care post treatment
-Consultation with other services as needed
-Provide acute/emergency care 24 hours a day 365 days a year
-Research initiative opportunities
-Optimizing patient safety and quality
Required knowledge Base:
- Fundamental management of acute and chronic medical conditions
- Recognition and proper utilization of consultative services
- Familiar with and employs diagnostic and therapeutic treatment plans that satisfy current standards of care
Additional valuable knowledge base:
- Knowledge of CPRS
- Knowledge of VISTA
- Knowledge of how to access imaging and lab studies
- Knowledge of standard navigation of the consult process
- Provides direct patient care as patient primary outpatient clinics
- Works with and is supported by Medicine Service leadership.
- Provides patients with accurate, compassionate, and cost-effective treatment.
- Responsible for providing services in an efficient, high-quality, cost effective manner within budget allocation.
- Works with the Chief, Medical Service and other staff as appropriate, supports operations issues/problems resolution in a logical, step-wise fashion. - Participates in a forum in which staff identify, communicate and resolve issues related to Medical Service, Medical Center ancillary and support operations and VISN-wide mandates.
- Works with the Chief, Medical Service and other disciplines as necessary to support, contribute to and implement policies, procedures, performance goals, strategic plans and initiatives.
- Seeks and enhances opportunities to promote and strengthen the Medical Center's and VISN goals. Responsible for implementing the goal of improving care for patients within the Medical Center and VISN.
- Has basic understanding of managed care practices, including health care finance, billing, coding and documentation.
- Has knowledge, ability and willingness to use and promote the dissemination among clinical staff of current automated technology for Health Information Management Sections (HIMS), patient data tracking, patient orders, etc.
- Contributes to the development, maintenance and enhancement of relations with internal and external entities including but not limited to: medical and nursing schools, therapist training programs, local public and private health care entities, colleagues in other Patient Services and Integrated Services such as Information Technology, Laboratory, and Business Office, labor, and VISN.
- Participates in Medical Service strategic planning, as requested.
- Participates in local administrative, management, quality assurance, and other assignments, meetings or committees, as requested.

Qualifications
To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR
[(2) Those approved by the American Osteopathic Association (AOA),OR
(3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences.
Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs.
Proficiency in spoken and written English.

- Participates in regular peer review activities.
- Must be alert to patient's condition and disorders. Reactions on the part of the patient require immediate attention so harm does not come to the patient.
- Maintains patient records and files. Follows HIPPA guidelines regarding patient information.
- Ensures timely completion of daily clinic notes with accurate documentation and coding of service rendered.
- Follows established safety practices and reports to Chief as promptly as possible to unsafe conditions, unsafe practices or any occupational illness or injury sustained. Adheres to all safety and health standards, regulations and work practices. Uses proper protective equipment as required.
- Participates in quality assurance (QA) activities to include and provide data collection, data analysis and trends, and reports on changes based on data collection.
- Hospital Identification Badge is worn in accordance with hospital regulations.
- Other duties as assigned.
Supervisory Controls
- The Primary Care Provider, Medicine Service Line (MSL), Primary Care/CBOCs reports to the Medical Director of the Oak Lawn clinic, Associate Chief, MSL, Primary Care, CBOCs/Hines, Chief, Medical Service, Chief of Staff, and Medical Center Director. Functions locally as a team member of the Medicine Service Line with a regular interface with the Chief of MSL.

- Age-specific care, as identified in the Age-Specific Competencies, including knowledge of changes associated with aging and principles of growth and development relevant to the adult and geriatric age groups; ability to assess and interpret data about the patient's status; and ability to identify age- specific needs and provides the appropriate care based upon the age related factors.
Computer Security
- Protects printed and electronic files containing sensitive office data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, Federal regulations, VA statutes and policy, and VHA policy. Protects the data from unauthorized release or from loss, alteration, or unauthorized deletion. Follows applicable regulations and instructions regarding access to computerized files, release of access codes, etc.
- Uses word processing software to execute several office automation functions such as storing and retrieving electronic documents and files: activating printers; inserting and deleting text, formatting letters, reports, and memoranda; and transmitting and receiving e-mail. Uses the Veterans Health Information and Technology Architecture (VistA) and the Computerized Patient Records System (CPRS) to access information in the Medical Center Computer System.
Environment Of Care
- Follows Life Safety Management (fire protection) procedures. Reports safety hazards, accidents and injuries. Reviews hazardous materials/Safety Data Sheets (SDS)/waste management. Follows Emergency Preparedness plan. Follows security policies/procedures. Complies with federal, state and local environmental and other requirements preventing pollution, minimizing waste, and conserving cultural and natural resources.
Infection Control
- Demonstrates infection control practices for disease prevention (i.e. hand washing, universal precautions/isolation procedures, including TB requirement/precautions.

Education
Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
- Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
- Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
- For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
