This position is located in Primary Care- Patient Aligned Care Team (PACT), responsible for the provision and coordination of services required for organized, multi-disciplinary team approach assuring quality, comprehensive care.
Duties
NOTE: The Licensed Practical/Vocational Nurse (LPN/LVN) grade is determined by education,
experience and verified qualifications as required by VA Handbook 5005/3, Part II, Appendix G13;

Standard.
Major Duties and Responsibilities
A. Knowledge and ability to appropriately carry out assigned Veteran care based on the Veterans'
conditions; to use judgment in selecting the appropriate order and sequence of procedures and
treatments; and to accurately recognize, report and record relevant Veteran information.
B. Completed work should need only a general review by a RN or Provider for appropriateness and
conformity with established policies and procedures.
C. Managing clinic workflow. Ensuring patients are placed in examination rooms in a timely manner
and providing direction to patients as they move through the clinic environment.
D. Assist in panel access through scrubbing weekly reviewing scheduled appointments.
E. Demonstrates the ability to observe, identify, and respond to the Veteran's needs for care,
including medication, equipment-assisted care, and Veteran/family education. In organizing and
delivering care, the LPN recognizes and considers emotional, cultural, spiritual, socio-economic,
and age-related factors.
F. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular
and/or intravenous) and performs treatments according to established policies/procedures.
G. Observes for physical and/or emotional changes in Veteran's condition from prescribed
medications/treatments, promptly and accurately documenting noted changes, and reporting any
deviations from normal to RN or Provider.
H. Uses sound judgment and demonstrates the ability to recognize the need for initiating emergency
measures when indicated and assist in response procedures for cardiac/pulmonary arrest,
anaphylaxis, fire, disruptive behavior, bomb threat, external disaster, missing patient, and
hazardous spills. Promptly seeks the assistance of the RN or Provider.
I. Exhibits knowledge and understanding of human behavior, individual motivation/reaction to
situations, population specific factors and demonstrates the ability to appropriately utilize this
knowledge in working effectively with Veterans, family and caregivers, and internal and external
customers.
J. Possesses knowledge and skill in performing support duties for complex diagnostic tests and/or
specialized practices or procedures, which include preparing the Veteran, assisting in the
diagnostic examination, preparing and handling specialized instruments or other specialized
equipment, and monitoring
the Veteran's condition before, during, and following the procedure.
K. Serves as a preceptor in orienting, educating, and training less experienced or
technically skilled staff related to support duties for these more complex, specialized
tests/procedures.
L. Actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing
new knowledge gained with other staff to improve and advance nursing practice.
M. Maintains flexibility in adapting to changes in schedule and workload. Assist other team members
willingly.
N. Safeguards patient privacy and maintains confidentiality of all electronic, written, verbal and
printed patient information according to HIPAA guidelines and the Privacy Act.
O. Demonstrates professional behavior (i.e., respect, civility, ability to accept constructive
criticism), effective communication and interpersonal skills.
P. Maintains a work environment that honors diversity.
Q. Adheres to principles of infection control, safety, and universal precautions.
R. Follows policy and procedures related to fire and disaster drills, safety and security measures.
S. Utilizes appropriate computer software packages including Vista, Outlook,
Microsoft Word, CPRS, VATAS, etc.

Qualifications
Basic Requirements:
United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed.
Licensure. Full, active, current and unrestricted licensure as a [ ] licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
Physical Standards. Per VA Handbook 5019 (Physical exam required for this position).
Certification: Must be current in BLS/CPR certification.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:(1) Qualifying experience is successful nursing practice [as an LPN/LVN,] maintaining acceptable standards within a health care setting under the direction of a registered nurse [(RN)] or physician [(MD/DO)]. This may include experience as a GPN [or GVN] (graduate practical [or vocational] nurse) provided the candidate was utilized as a practical or vocational nurse and passed the State licensure examination on the first attempt.(2) To be creditable, practical nursing experience (as an LPN or LVN) must be documented on the application and verified in an employment reference or through other [independent] means.(3) Part-time experience is credited according to the relationship it bears to the full-time workweek (e.g., an LPN[/LVN] who worked 20 hours per week, i.e., half-time, would receive 1 full-time workweek of credit for each 2 weeks of such service.)To qualify at the announced level (GS-6) you must meet these additional requirements:
Have at least one year of related experience at a high level of performance reflecting the following knowledge, skills, and abilities.
Able to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Provides effective education to patients and/or family members in relation to common disease
Demonstrated ability to serve as a responsible member of the nursing team and interact with patients, family members, professional and supportive personnel.
Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures.
Knowledge and ability to recognize the need for and to institute emergency measures, promptly seek assistance and assist in resuscitation procedures in cardiac and/or pulmonary arrest.
Recognizes and responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, prompt reporting.
In addition to the above:
Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations.
Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant] patient information.
Ability to observe, identify and respond to the patient's needs.
