The Eastern Colorado Health Care System is currently recruiting for one GS 12 Senior Social Worker for the Housing Urban Development-Veterans Affairs Supportive Housing, HUD-VASH Program.
Duties
Senior Social Worker-HUD-VASH duties include, but aren't limited to the following:
Provides case management for HUD/VASH program for chronically homeless Veterans & provides psychosocial services to Veterans/families while residing in stable housing through HUD's Section 8 voucher system
Serves as a Senior Social Worker, specializing in Geriatric and Extended Care, GEC for aging Veterans/their families
HUD-VASH program combines HUD HCV rental assistance for homeless Veterans, some of whom may be at elevated risk for suicidal thoughts/behavior, w/case management & clinical services provided by Department of Veterans Affairs, VA at Medical Centers & Community Ongoing VA case management, healthcare & supportive services are available for homeless veterans using approximately 1,900HUD-VASH vouchers
Provides comprehensive social work services to homeless Veterans/families participating in HUD-VASH programs & ensures Veteran's care is appropriately coordinated w/ VA Medical Facility, community/mental health providers & agencies that serve homeless populations
Acts as liaison between VA & community resources
Consults for HUD-VASH social workers who serve Veterans with complex mental health, medical, co-morbid disorders or who display increased risk for mild to moderate disruptive behavior & adheres to NASW Code of Ethics
Trains staff on coding, CPRS documentation & best practices
Independently manages more complex cases and accompanies other SW staff on home/community visits when Veteran has a complex presentation to train less experienced staff
Performs one or more of the following: process improvement tasks, social worker supervision for advanced practice licensure/graduate trainee program, acts as mentor for less experienced social workers, participates/chairs committees impact social work practice &/or leads roll out of evidence-based practices.
Provides ad hoc coverage for Supervisor in regard to CPRS: cosigns notes, consults, orders & provides clinical guidance
Reviews agreements between Section 8 landlords, Public Housing Authorities & VA & establishes referral sites for HUD-VASH placements
Performs HUD-VASH apartments site visits to ensure Veterans reside in safe environments, in compliance w/ local housing codes
Verifies Veteran's status/eligibility for VA medical care/program participation
Collects & submits HUD-VASH Program participant data outlined by NEPEC evaluation procedures & HOMES
Serves as Veteran's advocate & ensures comprehensive service delivery, links/refers Veterans to VA Medical Center, VA Regional Offices & community agencies
Complies w/ criminal conflict of interest laws & Executive Branch Standards of conduct
Follows VA, federal, local, CARF & JCAHO policies, SOPs & documentation requirements for Medical Center & HUD-VASH Program
Plans, develops, advises of shifting trends in caseloads & identifies needs/makes recommendations for program procedures/policy changes
Maintains data/statistical compilations to comply w TJC, CARF, VA & medical center policy/procedures
Teaches Veterans/families to cope w/stages of change, loss/grief experiences due to disability, terminal illness & death and ensures appropriate hand off when transferring of patient care
Is familiar/responsible for all performance metrics relating to HUD VASH Program
Travel is required & may transport Veterans in government vehicles to meet Veterans' treatment goals
Demonstrates I-Care values when interacting w/ all internal/external customers
Maintains current knowledge of & practices in accordance w/ The Joint Commission requirements
Protects printed & electronic files containing sensitive data according to Federal privacy Act & confidentiality statutes
Follows regulations & instructions regarding release of sensitive/protected patient/ employee information
May serve on Service Line/VISN 19/medical center committees, workgroups, &/or task forces, etc.
Performs other duties as assigned
Work Schedule: Monday-Friday 8:00am-4:30pm, rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs on weekends

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Applicants must be proficient in spoken & written English
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to the social worker position GS-0185 series in the VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/ORHM//T38Hybrids.
Loss of Licensure or Certification. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determination Requirements: The following criteria must be met in addition to the Basic Requirements for employment listed above when determining the candidate's grade.
Senior Social Worker, GS-12
Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all the following KSAs:
1) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
2) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
3) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
4) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
5) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
