This position is located at the Jesse Brown VA Medical Center, Chicago, Illinois and is organizationally aligned under the Office of the Chief of Staff, Geriatrics and Extended Care Service (GEC). The Program Support Assistant provides administrative support to the manager and staff of Home and Community Based Care Programs (H&CBCP).
Duties
Request medical documentation for community health services verbally and in writing.
Performs follow up calls when documents are not forthcoming.
Documents agency contacts (three times minimum) in CPRS Consult Toolbox.
Receives all documentation, verbal order confirmations, change orders, etc.) from home care agencies in the community requiring physician signatures.
Sorts and distributes documents to physicians across the medical center and community-based outpatient clinics (CBOCS).
Maintain tracking sheets to document receipt, distribution chain, and return of documents from agencies and physicians.
Completes GEC consults by completing the appropriate note in CPRS consult toolbox and scanning all Home Health Certification and Plan of Care - documents that include physicians' signature into the Vista Imaging program assuring the reports are linked to the appropriate consult, progress note title, and patient.
Ensure documents are returned to the agencies as appropriate.
Generate a report monthly showing all patients who require a 180-day review.
Complete 180-day personal care services reviews on all Veterans enrolled in patient care services (Homemaker/Home Health Aide, Respite, Veterans Directed Care) programs.
Communicate to RNs any requests for agency changes, adjustments to frequency and duration for action, patient deaths, and discontinuation of service.
Creates and maintains budget spreadsheet for tracking Veterans Directed Care spending plan monthly.
Maintain all documents generated by the third-party administrator and scans reports into the patient chart when the patient passes.
Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/14/2023.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

You may qualify based on your experience as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade (GS-05) in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Experience requesting and receiving medical documentation; ability to maintain, document receipt and return documents; knowledge of CPRS; ability to generate reports based on stored data; ability to monitor established tracking systems and ensure the entry of information into data tracking systems; ability to facilitate staff meetings; skill in screening phone calls in a courteous and timely manner; knowledge of commonly used medical terminology.

Physical Requirements: No special physical demands are required. The work requires the ability to move around the office and medical center and carry light items such as medical carts, folders, and papers. Some bending and stooping may be required during the course of work. The work is mostly sedentary.
