Roseburg VA Health Care System is hiring for their downtown Eugene Community Based Outpatient Clinic (CBOC). The Registered Nurse (Health Promotion Disease Prevention-HPDP/Veteran Health Education Coordinator-VHEC) coordinates health promotion and disease prevention efforts across the facility/healthcare system, including CBOCs. Health promotion refers to informational, motivational, and clinical activities designed to encourage improvement in the health and well-being of individuals.

The duties of the RN Health Promotion Disease Prevention (HPDP)/Veteran Health Education Coordinator (VHEC) include, but are not limited to:
Advises facility and primary care leadership on clinical and administrative issues related to HPDP, including clinical preventive services.
Provide coordination of training for TEACH program with assistance from Health Behavior Coordinator (HBC) and provide assistance to Health Behavior Coordinator to provide Trainings in Motivational Interviewing and Clinician Coaching.
Demonstrates leadership, experience, and creative approaches to the management, delivery and maintenance of the Veterans Health Education Program.
Leads review, vetting, and approval of in-house created Veteran Health Education materials.
Designs, develops, and facilitates HPDP outreach events including strategies to evaluate outcomes based on the best available evidence.
Develops or coordinates the development of user-friendly tools to assist in successful implementation of HPDP programs and services.
Functions as a designated focal point, coordinator, advocate, and liaison for Community patient education, wellbeing, self-management, and CIH programs.
Serves as the principal point of contact for all Veterans health education communications and reporting within the facility.
Maintain effective relationships with PACT and Specialty Teams to Network and best support these teams.
Routinely assesses the prevention-related educational needs of both patients and employees, develops educational programs and learning outcomes, coordinates the development, delivery and evaluation of those programs and recommends and implements changes to the thee program or service level to meet HPDP standards.
Collaborates with primary care and all clinical and administrative services as needed to ensure that HPDP programs and services are available and maintains effective working relationships with other facility staff responsible for related programs and services, including the Health Behavior Coordinator (HBC), MOVE!, Veterans Health Education, Women Veterans Program, Quality Management, Public Affairs, Social Work, Voluntary Services and Others.
Serves as the facility liaison to the VISN, VHA National Center for Health Promotion and Disease Prevention (NCP), and VHA Central Office program office in matters related to HPDP.
Works with facility management to assure that the Veterans Health Education Program contributes to the facility's mission, goals, and objectives.
Provides consultation and training to interdisciplinary staff members regarding health education theory and practice, evidence-based health education interventions and skills, and health education program development to assure that Veterans health education programs, activities, and services meet quality standards.
Leads efforts to identify HPDP challenges, priorities for improvement, and opportunities for disseminating and sustaining best practices.
Provides regular reports to facility management on the status of Veterans Health Education programs, services and products.
Supporting managers and upper level management through reports and services.
Demonstrates leadership, experience, and creative approaches to the management, delivery and maintenance of the Veterans Health Education Program.
Demonstrates leadership, experience, and creative approaches to the management, delivery and maintenance of the HPDP Program.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: Program Management, Facilitation of Training. Training or certification in any Tier One CIH Modality. Such as Tai Chi, Yoga, Battle Field Acupuncture, Guided Imagery, Qigong. Training or experience in Health Literacy. Experience working with Chronic Disease Management such as Diabetes Educator, Blood Pressure management and Chronic Pain Management. Experience working with the MOVE! Or Weight Management Programs. MSN or Doctorate in Nursing with 2-3 years experience.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.
