The incumbent will serve as an administrative official in one or more of the HBPC, Home Based Transitional Care (HBTC) and MFH programs. The Deputy Program Director of HBPC, assists the HBPC Program Director and/or HBPC Medical Director(s) with program management and oversight of programs under Geriatrics and Extended Care Service.

Duties

The incumbent will serve as an administrative official in one or more of the HBPC, Home Based Transitional Care (HBTC) and MFH programs. The Deputy Program Director of HBPC, assists the HBPC Program Director and/or HBPC Medical Director(s) with program management and oversight of programs under Geriatrics and Extended Care Service. All programs are intended to serve as clinically appropriate, cost effective and efficient alternatives to lengthy hospital and other institutional stays/placement. HBPC, HBTC, and MFH enables chronically and terminally ill veterans to receive comprehensive health and supportive services in their homes. The Deputy HBPC Program Director reports to the HBPC Program Director, who reports to the Associate Chief of Staff, Geriatrics and Extended Care Service. The incumbent assists the HBPC Program Director administratively and programmatically to oversee a diverse professional staff and provides leadership, advice, counsel, and instructions to individual staff members on administrative and programmatic matters to improve care and clinical program outcomes. The professional staff includes Advanced Medical Support Assistants, Geriatricians, Psychiatrist, Psychologists, Advanced Practice Providers, Registered Nurses, Licensed Practical Nurses, Kinesiotherapists, Clinical Pharmacists, Registered Dietitians, and Social Workers. These functions are within established guidelines and demonstrates the ability to apply critical thinking logic and experience to work effectively and develop sound relationships with the interdisciplinary team, peers, other professional, clients, their caregivers, and supportive personnel. The Deputy HBPC Program Director is responsible to provide assistance, collaboration, and subject matter expertise for a complex group of services HBPC often works with, such as Home Oxygen Programs, Home Medical Equipment, SCI Home Care, MHICM, BROS and Prosthetics department to comply with regulatory standards. He/she is responsible for evaluating program activities and providing follow up actions required by internal and external review organizations and or accrediting bodies. As a leadership member of the Geriatrics and extended Care Service, he/she participates in the formulation of overall service/program policies and procedures, goals, objectives, strategic planning, cost effectiveness, and quality monitoring. Duties include but are not limited to: - Manages all aspects of the program for the facility, assuring local compliance with national policy and direction. - Coordinates with the multidisciplinary team to develop local goals. Measures progress toward the goals and reports to the appropriate officials at a local, VISN and national level. - Provides direct patient care by developing and implementing comprehensive nutrition therapy, practicing within their scope/clinical privileges. - Serves as the professional and technical advisor for their program. - Promotes and maintains effective working relationships with all faculty and significant parties at the national, state, and local level. - Directs or participates in the design of studies, collects data, conducts statistical analysis, interprets findings, and translates results into written and oral communications. - Sets priorities, delegates tasks, meets multiple deadlines, analyzes organizational problems, and develops and implements effective solutions. - Serves as a consultant and technical advisor within NFS and with other facility health care staff in evaluating health care delivery to Veterans and employees. - Determines the continuing education needs of program staff and appropriate resources to meet those needs. - Oversees all aspects for the program, policy, and procedure development. - Oversees development and maintenance of training and reference materials. - Establishes and manages program goals and objectives. - Plans and administers a program, coordinates associated activities and resources, evaluates program accomplishments, redefines priorities, problem solves, and modifies objectives. - Takes an active role in the fiscal management for the program, identifying resource needs that may include staffing, supplies, contracts, etc. - Maintains program information through approved social media outlet. - May perform other related duties as assigned which are appropriate for this grade and assignment. Other Significant Information The incumbent will need to drive a car and have a valid state license. The position often places the incumbent in a position of stress. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Citizenship. Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified candidates in accordance with chapter 3, section A, paragraph 3g) English Language Proficiency. Dietitians must be proficient in spoken and written English as required by 38 U.S.C. 7402(d). Education and Supervised Practice. The individual must have earned a bachelor's degree from a U.S. regionally accredited college or university or foreign equivalent and fulfilled each of the following criteria: Completed a didactic program in dietetics accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND), formerly known as the Commission on Accreditation for Dietetic Education (CADE). Completed an ACEND accredited or approved supervised practice program. Supervised practice programs are post-baccalaureate degree programs that provide supervised practice experiences which meet the eligibility requirements and accreditation standards of ACEND, formally known as CADE. Completion of a coordinated program in dietetics fulfills the requirements of a supervised practice program. Time spent in a dietetic internship or supervised practice program does not qualify as creditable experience. Individuals who have earned college and/or university degrees outside the United States and its territories also must have their transcripts evaluated by a foreign transcript evaluation service to determine if their degree is equivalent to a degree granted by a regionally accredited college or university in the United States. Validation of a foreign degree does not eliminate the need for verification of completion of didactic program, dietetic internship, or coordinated program requirements. The completion of ACEND didactic program requirements and at least a bachelor's degree from a U.S. regionally accredited college/university or foreign equivalent are necessary for eligibility to enter an ACEND-accredited supervised practice program. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Registration/Certification. All applicants must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA). Advanced Level Practice Dietitians must possess a minimum of one advanced practice credential relevant to the area of practice, (e.g. Certified Nutrition Support Clinician (CNSC), Board Certified Specialist in Renal Nutrition (CSR), Board Certified Specialist in Gerontological Nutrition (CSG), Certified Diabetes Educator (CDE), or Board Certified Specialist in Oncology Nutrition (CSO)). Grade Determinations: Experience. Completion of 1 year of experience equivalent to the next lower grade level. Preferred Experience:: 3 or more years as Program Director, Program Manager, Assistant Director/ Manager, or supervisor experience Assignment. . For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty) and variety and be performed by the incumbent at least 25% of the time. Employees in this assignment serve as Facility Dietitian Program Coordinator leading a multi-disciplinary team. Dietitian Program Coordinators may provide direct patient care by developing and implementing comprehensive nutrition therapy, practicing within their scope/clinical privileges. They are administratively responsible for a specialty program and provide program-specific leadership, guidance and direction, technical advice and staff development. Dietitian Program Coordinators oversee responsibilities for program, policy and procedure development, set goals, monitor and redefine priorities and may have an active role in the fiscal management for the program. They direct or participate in the design of research activities or quality improvement studies, including data collection and analysis. Dietitian Program Coordinators have oversight of and are responsible for developing and maintaining training and reference materials. They maintain program information through approved social media outlets. Dietitian Program Coordinators may supervise up to two employees. This assignment must represent substantial additional responsibility over and above that required at the full performance grade level and cannot be used as the full performance level of this occupation. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate the following KSAs Knowledge of program administration establishes program goals and objectives, coordinates activities, evaluates accomplishments and solves problems. Skill in developing quality improvement or research activities. Skill in developing training and reference materials. Ability to develop policies and procedures. Ability to provide program-specific coordination to include leadership, technical advice and staff development.
