A clinical pharmacist handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care.

The clinical pharmacist is required to be able to demonstrate a knowledge of current pharmacy laws, regulations and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. The clinical pharmacist is responsible for resolving a variety of questions posed by physicians, nurses and coworkers related to drugs, pharmacy services and departmental policies and procedures. Must be able to interpret VA, Federal and State regulations, Joint Commission, DEA, FDA, Medical Center and Pharmacy department policies and be able to apply the same to given situations. Must have the ability to exercise sound judgment in a variety of situations, and be able to refer matters of policy making to a supervisor.
The incumbent is required to maintain current knowledge and demonstrate professional pharmacy practices. The incumbent is required to complete all education and training requirements by the VA, VISN and Medical Center within the due date or as required by the Director of Pharmacy.
Clinical activities in this area including monitoring for drug-drug interactions, drug-lab interactions, and drug-food interactions. The pharmacist also reviews the patient profile for therapeutic duplications and potential adverse drug actions.
Requires the application of professional knowledge and judgment in interpretation and clarification of parenteral drug orders. The compounding of IV admixtures requires a specialized body of knowledge involving compatibilities and stabilities of parenteral drugs in various solutions.
Responsible for facilitating preparation and delivery of all compounded intravenous fluids as ordered by the physician.
Clinically, the staff pharmacist must be knowledgeable in therapeutic application of the various products dispensed including: parenteral drugs, intravenous solutions, hyperalimentation solutions, investigational drugs, oncology drugs and other items such as blood components and solution administration sets.
Promotion of the most effective use of medications and works with the practitioners to decrease medical center costs through reduction of nonformulary medications and assure that medications are written according to approved guidelines/protocols.
Therapeutic monitoring of specific medications or patient groups as requested by providers.
In the actual interpreting and filling of prescriptions, the pharmacist is expected to follow pharmacy policy as it pertains to dispensed quantities, prior authorization drugs, restricted drugs and appropriate auxiliary labels and patient medication information (PMI) sheets.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT).
Licensure: Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice.
Knowledge of professional pharmacy practice.
Ability to communicate orally and in writing to both patients and health care staff.
Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves: Must be in overall good health. The work requires standing, walking, moderate lifting and carrying of items. The work also requires the use of fingers, the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other), and the ability to hear (aid permitted).
