This position is located in the Health Information Management Section of Patient Administration Service of the VA Medical Center in Madison, WI.
Duties

Performs the conversion of medical documents not amendable to direct entry (i.e., advanced directives, flow-sheets, consents, ICU records, etc.), and numerous medical reports from non-VA facilities, private physicians' offices and internal departments;
Indexes each captured/scanned document within the appropriate electronic document management system (EDMS), associates each captured/scanned document with the correct patient's Computerized Patient Records System (CPRS) record, selects the appropriate document, assigns the appropriate description and nomenclature, and electronically signs scanned documents;
Opens mail and prepares, scans, and indexes records for fee basis patients;
Reviews records scanned into VistA Imaging for duplicate records, and attach medical records to non-VA/Choice consults;
Reviews and researches medical reports from non-VA health care facilities and makes a determination as to which documents are to be scanned, and obtains guidance from providers and/or Medical Records Committee's guidelines;
Ensures that all necessary health/administrative records, reports, and information are integrated into VistA through PC scanning software and indexes this information using PC/VistA application bridges;
Combines and/or repairs records as indicated by established guidelines to ensure that records are delivered in satisfactory condition;
Prepares files containing medical and administrative records for record retirement;
Generates the computerized scheduled list from Record Tracking (Death Records);
Performs a variety of miscellaneous administrative duties;
Answers any walk-in or telephone inquiries by patients/clients, employees, and volunteers in a timely and courteous matter;
Performs related duties as required.
Work Schedule: Monday through Friday, 8:00am - 4:30pm.

Qualifications
TYPING PROFICIENCY: This position requires the skills of a qualified typist. In addition to meeting the experience and/or education requirements, you must be able to type at least 40 words per minute. ~AND~
MINIMUM QUALIFICATIONS GS-04: Please select the appropriate answer to the following question regarding your level of experience and/or education to meet the minimum qualifications of this position.
A. One year of specialized experience that equipped you with the knowledge, skills, and abilities to successfully perform the duties of this position, and that is typically in or related to the position. To be creditable, specialized experience must have been equivalent to at least the GS-03 grade level in Federal service. Specialized experience includes: performing clerical duties in support of an office; entering data into databases; and communicating effectively to provide courteous and efficient customer service. This experience must be detailed in your resume to receive credit. ~OR~
An associate's degree or 2 years of successfully completed education above the high school level in any field for which high school graduation or the equivalent is the normal prerequisite. This education must have been obtained in an accredited business or technical school, junior college, college, or university for which high school graduation or the equivalent is the normal prerequisite. One year of full-time undergraduate study is defined as 30 semester hours, 45 quarter hours, or the equivalent in a college or university or at least 20 hours of classroom instruction per week for approximately 36 weeks in a business or technical school. Physical Requirements: The work is mostly sedentary but some bending, stretching, walking, and lifting are required. Boxes of records weighing up to 50 pounds must be packaged and lifted for shipment. Work is normally performed in an office setting.
