The incumbent is the Supervisory Social Worker of the Community Nursing Home/Adult Day Care /State Veterans Home (CNH/ADC/SVH) Program, for the Carl Vinson VA. The incumbent is delegated the responsibility and authority for the planning and coordination of all elements which constitute the CNH/ADC/SVH program. This requires a high and varied level of professional expertise and collaboration to develop, revise and/or apply policies and standards which determine the scope of the CNH/ADC/SVH.

Duties

The Supervisory Social Worker is responsible for the professional and administrative management of an area in social work service that coordinates Community Nursing Home/Adult Day Care Services. As the supervisor, the incumbent assists in planning, establishing, and implementing/adhering to policies and procedures; developing individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating and overseeing the staff GS -11 Social Worker, RN II-III that manage Veterans placed in the CNH facilities and the GS -5, Program Support Assistance, GS-7 Program Support Assistant within the CNH program. Incumbent ensures that the employees understand and adhere to the program directives, policies and standards, as well as VAMC employee requirements. The incumbent is responsible for scheduling and assuring that the annual VA CNH Exclusion Reviews are completed in the required timeframes as set forth in VA policy. This includes ensuring community nursing homes under Contract or Provider Agreement comply with Life Safety management (fire protection) codes and procedures. Must have a good knowledge of Centers for Medicare/Medicaid standards concerning CNH programs. This includes updating the CNH official web page with current data on the status of each CNH as required by VA directives. He/she is responsible for assuring that monthly follow-up to Veterans in the CNHs are completed as required by the program Social Worker and the Registered Nurse assigned to provide CNH follow-up. The incumbent is responsible for the oversight of CNH Program and provides care coordination (admissions, treatment planning, discharge planning, etc) and monitoring for Veterans in the CNH Program within stated timeframes. He/she monitors documentation to ensure it is completed after each visit in the Veterans VA Medical Record using CPRS. The incumbent is responsible for overseeing the care of Veterans in the CNH Program and assuring that placements meet VA eligibility requirements. The incumbent is responsible for assuring that Quality Assurance monitors related to these programs are completed and reported as required: MDS QI Monitor, Patient/Family Satisfaction Monitor, Timely Visits and Documentation Monitor, and Sentinel Event Monitor. The incumbent is responsible for reporting and investigating Sentinel/Adverse Events to Central Office via the VISN liaison for the CNH Program. The incumbent works independently in providing appropriate documentation, review and follow-up of complaints within the CNH Program. This includes follow-up on substantiated complaints against the community nursing homes, which are investigated by AHCA. Reviews of the AHCA Watch List are conducted and changes in status are reported. He/she meets and maintains communication with the local ombudsman to discuss concerns of mutual interest. He/she is also responsible for organizing and preparing reports concerning the CNH Program for the Chief, Social Work Services, Medical Center Director, VISN liaison, and Central Office. The incumbent attends and or participates in a variety of monthly and quarterly conference calls, disseminating needed information to VA staff and community agency staff as warranted. The Social Worker will fulfill ethical responsibilities to Veterans, colleagues and VA in accordance with relevant discipline Code of Ethics, VHA Code of Ethics, and local Ethics policies, which includes but is not limited to compliance with all laws and regulations related to confidentiality and release of information and protecting Veterans' personal identifying information. Work requires regular and recurring periods of standing, walking, lifting, accessing transportation, and driving a government car or van. While this is primarily a clinic-based position, there may be times when this position requires work in the community which may include the ability to operate a government vehicle and transport passengers. Work will be performed in a wide range of settings, including within the clinic; in Veteran's homes; in community-based settings; or in transport vehicles. Work areas are often noisy, irregular, and unpredictable, and can be stressful at times. Community-based visits should be made with safety in mind and follow the Home Visit Safety policy. Qualifications

Basic Requirements: Citizenship. Be a citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Please note: A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Regulations. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Must pass a background check. Grade Requirements: GS-12 Supervisory Social Worker Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. At the GS-12 grade level, supervisory social workers typically have significant personnel management responsibilities for a small to moderate sized professional and non-professional staff, including support staff and professional staff at the GS-12 grade level and below. The supervisory social worker is responsible for the professional and administrative management of an area in a social work service, in a care/product line, or multiple facilities. They provide direct clinical services with or without program coordination responsibilities.] Experience/ Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. License. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: 1. Ability to independently organize work, set priorities, and meet multiple deadlines. 2. Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. 3. Ability to ensure provision of clinical social work services. 4. Ability to provide staff training and development. 5. The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is a GS-12. Physical Requirements: Physical demands of the work are generally minor. The work is primarily sedentary.
