This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

Provides clinical services and case management to Veterans admitted to the Domiciliary. Clinical case management services will include, but not limited to, monitor continued progress in treatment services, access to psychiatric services, access to vocational services, access to housing options, and transition services. Provides evidence based therapeutic interventions in encounters with Veterans to promote recovery and successful transition to community living and care. Therapeutic interventions will be specifically delivered in treatment modalities for addictions, substance abuse and dependence, complex mental health conditions, and PTSD. The majority of their work will be to conduct group therapy interventions and some individual therapy to support these specialty modalities as needed in a domiciliary environment. Designs, plans and conducts various psycho-social educational classes and process groups related to helping the veterans understand mental health and their needs and the impact on them and their whole life system of support. Collaborates with the Domiciliary Chief, other Program Managers/Coordinators, and other staff on the development of programing specific to addictions to include coverage, viability, and adjusting as new evidence for interventions are presented. Conducts psychosocial assessments to determine needs of veterans and family members and drive treatment planning. Participates in the treatment planning process with other disciplines. Coordinates and refers care with other disciplines and will be responsible for ensuring that the treatment plan is executed in a manner that is timely and appropriate. Provides individual, family and group counseling to support treatment and recovery of Veterans experiencing addictions, complex mental health conditions, and PTSD. Provides consultation/education to veterans and families on community resources, advance directives and VA benefits. Acts as a consultant to other team/staff members as well as take input from other disciplines in the consultative process. Facilitates VA and community resources, to include marketing and public relation duties as needed. Provides case management to veterans. They will engage family and other supports to promote recovery for Veterans. Provides a full range of social work services within commonly accepted standards of social work practice which includes case work/case management, individual, family and group counseling, discharge planning, and other services as appropriate. Supervises the practice of students, or a less experienced Social Worker Assists the Veteran and family in understanding contributing factors to problems, discuss solutions, and encourage lasting changes to reduce stressors. Participates in committee assignments, as deemed necessary by the service manager. Maintains data and statistical compilations to comply with The Joint Commission, VA and medical center policy and procedures. Documents therapeutic interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. Loss of Licensure or Certification. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. Physical Requirements. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
