This position is in the Cardiac Cath Lab, Nursing Service, Boise VA Medical Center, Boise, ID. This lab provides cardiac procedural diagnosis and intervention for both inpatients and outpatients. The Cardiac Catheterization Technician works closely with cardiologist, clinicians, technologist, nurses and clerical staff to assure optimum quality control, efficient coordination of activities, high quality diagnostic/therapeutic exam results, and patient satisfaction.

Duties

- Incumbent triages exam request to give priority to cases that are clinically more urgent and coordinates the completion of impatient exams with scheduled outpatient exams. - Assures all equipment is operational prior to patient procedures, contacts Cath Lab Supervisor/ Biomed / Attending Physician when equipment is inoperative or malfunctioning. - Secures patient's confidence/ cooperation during procedures. Explains the procedure, positions patient for desired results, cushions and shields patient for a variety of difficult procedures. - Properly incorporates appropriate PPE when conducting procedures. - Gathers patient information from the electronic medical record. - Operates cardiac physiologic monitoring equipment. Sets up and calibrate pressure transducers. Properly identifies, labels and samples intracardiac and vascular waveforms. Completely and accurately documents all aspects of the procedure including capturing and inputting data or procedural calculations in the hemodynamic monitoring system. - Preps and drapes clients for invasive/ operative procedures. - Facilitates insertion and manipulation of guidewires and catheters into vasculature under fluoroscopic guidance for diagnostic and interventional purposes. - Obtains invasive hemodynamic data, cardiac output, and blood samples. - Injects contrast manually or with the aid of a mechanical contrast device to visualize anatomical structures (angiography, venography, etc.). - Administers intra-coronary medications via manifold system during angiography procedures in accordance to physician orders. - Operates single plane fluoroscopic equipment utilized in imaging. Manipulates imaging equipment to optimally define anatomy. Selects magnification acquisition techniques specific for procedures and anatomy and performs post processing of acquired images including quantitative analysis, digital subtraction and image annotation. - Sets up and operates ancillary procedure equipment including the power injector, IVUS, FFR/IFR , Atherectomy device, Intra-Aortic Balloon Pump, Xper system and lntellispace image archive system. - Performs Activated Clotting Time (ACT}, Whole Blood Oximetry and Blood Glucose Monitoring quality control and patient testing using Point of Care equipment in the Cath Lab. Follows CAP Survey Standards of the General Laboratory. - Coordinates bed control and unit activities. - Transports inpatients and outpatient to and from Cath lab. Transfers patients back and forth between stretcher/ ICU bed and procedure exam table. - Incumbent preforms a variety of personal care or nursing care activities that do not require practical or vocational nursing licensure, or professional nurse education. - Assists with patient care and operate specialty equipment during clinical emergencies ( cardiac arrest, tamponade, hypotension, shock etc.). - Stocks rooms and monitors inventory PAR levels and notify to re-order. - Checks expiration dates of lab stock weekly. - Enters data in daily log and enters event capture and prosthetic charges. - Clinical orientation of new employees. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. See the grade requirements part of this standard for information regarding educational substitutions. Licensure or Certification: Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-9 Medical Instrument (Cardiac Catheterization)In addition to the core competencies defined at the GS-8 full performance level, positions at this grade may be lead, supervisory, or non-supervisory with specialized advanced knowledge and assignments . Experience. At least 1 year of experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be experience which demonstrates the knowledge, skills, and abilities to fully perform all cardiac catheterization tests and examinations and to function as a technical director of the cardiac cath lab and provide oversight and/or guidance to other technicians. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): Demonstrated Knowledge, Skills, and Abilities. KSAs 1, 2, and 3 are appropriate for all positions at this level. KSAs 4 and 5 are to be used in accordance with their appropriateness for the specific assignment. 1. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to vascular ultrasonography. 2. Knowledge of JCAHO and other regulatory requirements. 3. Ability to communicate effectively and professionally with employees at varying grade levels. 4. Ability to manage and supervise employees. 5. Ability to provide, or provide for, staff development and training. Certification. Registration by Cardiovascular Credentialing International (CCI) as a registered Cardiovascular Invasive Specialist (RCIS) is highly desirable. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable for employees at this level. Preferred Experience: Cath Lab experience is preferred but not required. References: VA Handbook 5005/15, Part II Appendix G-[27], Medical Instrument Technician Qualification Standard. This can be found in the local Human Resources Office. The full performance level of this vacancy is GS-09. Preferred Experience: Two years successful clinical experience assisting in diagnostic and interventional cardiac and vascular catheterization procedures and cardiac device implants (pacemakers, implantable cardiac defibrillators) as first scrub assistant, cardiac hemodynamic recording system operator Cath Lab circulator, and single plane x-ray operator in the Cardiac Catheterization Laboratory. Knowledge of the more complex examinations and treatment procedures and techniques in order to provide training and supervision in theses procedures. BLS and ACLS required. Vascular catheterization experience is highly desirable. References: VA handbook 5005, Part II, Appendix G27, Appendix B. The full performance level of this vacancy is GS-09. Physical Requirements: Physical aspects associated with work required of this assignment are typical for the occupation, see Duties section for essential job duties of the position.
