The Clinical Pharmacy Technician will provide a broad range of professional and clinical pharmacy support in a complex and geographically vast health care system. The Clinical Technician will be an integral part of our new strategic plan to implement a targeting strategy with Primary Care. They will be the primary person in charge of population management (getting patients into clinic) while the Clinical Pharmacy Specialists (CPSs) actually are conducting the visits in clinic.
Duties
The incumbent is a highly specialized, efficient, motivated and customer-friendly technician, who, under the authority of the Chief, Pharmacy Service, is responsible for the technical and appropriate management of Non-VA Care prescriptions. The incumbent is expected to work with considerable autonomy but under the guidance of a registered pharmacist and the under the direct supervision of a pharmacist supervisor as required by law.
Typical duties include, but are not limited to:
1. Provide support to clinical pharmacist practitioners as assigned according to approved clinical pharmacy protocols.
2. Utilizes population management (eQM) reports to identify patients who may benefit from PharmD Medication Management services.
3. Coordinate scheduling of pharmacy clinic patients and assist with lab ordering.
4. Communicates lab results to patients as deemed appropriate by clinical pharmacist practitioner(s).
5. Conducts patient interviews, including medication history, anticoagulation history, and effectiveness of anticoagulation, need for medication refills, and investigating causes of therapeutic irregularities.
6. Routinely review computer medication profile for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies/adverse drug reactions. Reports identified issues to a pharmacist for further action.
7. Routinely reviews laboratory monitoring reports for selected disease states and identifies patients with lab values beyond normal limits. Reports identified issues to a pharmacist for further action.
8. Places orders for CPS verification.
9. Routinely performs patient medical chart, medication profile, and action profile reviews to identify poly-pharmacy situations. Reports identified issues to a pharmacist for further action.
10. Documents all patient specific clinical decisions (in consultation with the clinical pharmacist) in the patient's electronic record.
11. Collects and documents patient vital signs.
12. Provides DOAC Dashboard management as assigned.
13. Coordinates CGM set-up and provides patient education.
14. Send patients information via mail or secure messaging as needed.
15. Attends and actively participates in all clinical pharmacy activities, i.e., staff development, residency conferences, journal club.
16. Participates in self-development programs, assigned meetings, and assigned teaching activities.
17. Serves on and acts as a consultant to professional committees as assigned.
18. Manages technical assignments of PACT & anticoagulation clinics in order to assure continuous and consistent coverage of the clinic.
19. Monitors patient scheduling to assure appropriate patient follow-up and time to next clinic availability.
20. Provide telephone triage and patient follow-up as assigned.
21. Utilizes VVC as assigned to communicate with Veterans.
22. Promote harmonious relationships within the work environment supporting the needs of the team and the Veteran.
23. Communicate and follow-up on requests in a timely manner based on the needs of the clinic team/supervisor.

Work Schedule: 8-hour shift between the hours of 6:00am-6:00pm, with rotating weekend, holiday, evening, night hours when required.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Pharmacy technicians appointed to direct patient care positions must
be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part.
Certification. Certification by the National Pharmacy Technician Certification Board (PTCB) is required at grades GS-6 and above.
Grade Determinations:

(a) Experience. One year experience at the next lower level and must fully meet the KSAs at the next lower level. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*):

7. Comprehensive knowledge of controlled substance policies and procedures, and the ability to identify and investigate discrepancies in these policies and procedures.
8. Comprehensive knowledge of, and ability to, analyze data in VA information systems.
