The employee serves as a Social Work Associate for the Veterans Healthcare System located in Ann Arbor, Michigan.
Duties

The Social Work Associate performs a wide variety of duties in support of professional social workers, provides assistance and services as prescribed for individual cases by social workers in conjunction with their professional treatment. interviews patients and others for informational or service purposes; handles referrals to social and health facilities; obtains and compiles data and other similar types of work.
Duties include, but may not be limited to:
Serves as initial point of contact and effectively interacts with patients, families, providers, and administrative personnel.
Develops psychosocial assessments of clients in order to identify emotional, social, physical, familial, economic and vocational/avocational needs and resources.
Interviews patients and others for informational or service purposes.
Handles referrals to social and health facilities.
Obtains and compiles data
Educates and assists patients and family members on the Advanced Directive (Power of Attorney; Living Will) and the abilities and limitations of such documents and refer to outside legal resources.
Enters and maintains guardianship documentation into the electronic health record.
Manages incoming and outgoing mail for the Social Work and Community Based Service (SW&CBS).
Tracks incoming consults to SW&CBS programming for Ann Arbor and 6 Community Based Outpatient Clinics (CBOCs).
Answers telephone to triage calls, questions, concerns, including urgent matters and determines appropriate action based on contact.
Provides direct referral to internal and external resources such as Voluntary Services, food resources, lodging resources, Patient Advocate, Veteran Service Officer, public transportation and other immediate needs.
Ensure fair and equitable treatment and delivery of services to all Veterans.
Coordinates with lead/supervisors to assure staffing and daily coverage.
Orients volunteer to department; and coordinates tasks for volunteers and ensures tools to complete volunteer work are in place.
Orders and maintains supplies and resource documents and pamphlets for social services.
Performs other duties or special assignments as assigned.
Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications
To qualify for this position, applicant(s) must meet all of the following:
ENGLISH LANGUAGE PROFICIENCY: In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position.
BASIC EDUCATION (TRANSCRIPT REQUIRED): Requires a bachelor's degree from an accredited college or university in major fields of study such as: social welfare, sociology, psychology, public administration, philosophy, education, anthropology, history, economics and/or English.
SPECIALIZED EXPERIENCE: Requires two full years of experience as a social welfare worker or social work assistant in a voluntary, public, or health agency. Experience must have been at a level of difficulty and responsibility equivalent to the next lower grade level (GS-7) in the Federal service. One full year of this experience must have been in a medical setting. Examples of specialized experience would typically include providing assistance and service support as prescribed by social workers in conjunction with patients' treatment plans; interviewing patients; obtaining and compiling data; and assisting with local, state or federal benefits and referrals for food, shelter, transportation, etc. (Resume(s) must clearly support the required experience and duties of the position. IMPORTANT: A full year of work is considered to be 35-40 hours of work per week. All experience listed on your resume must include the month and year start/end dates. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Physical Requirements: The work activity is primarily sedentary in nature: personal interviews, consultation with other staff, telephone calls, desk work, etc. Some walking to other areas of the medical center is required to facilitate patient contact or pursue planned activities. The pace of work is fast paced with competing demands.
