This floating LPN position, will travel to Chillicothe, Lancaster, Portsmouth and Wilmington Primary Care locations and is accountable for ensuring the delivery of safe nursing care for a group of patients based on their role in the nursing process, taking into consideration each patient's developmental stage, progressing from young adult to geriatric stage. Incumbent is expected to comply with all mandated review requirements and rotate to clinical areas as assigned.
Duties
Duties include, but are not limited to:
Delivers fundamental, knowledge-based care to assigned clients while developing technical competencies.
Works in collaboration with the registered nurse and/or providers from the Patient Aligned Care Team (PACT) within their scope of practice, according to policies, training and accepted VA practice.
Functions under the supervision of a Registered Nurse and/or provider and is directly responsible to the PCL Manger.
Works as a vital team member in collaboration with the PACT team and the ancillary departments to provide excellent quality patient care.
Collects and accessions specimens; prepares specimens for various automated urinalysis.
Obtains blood samples by venipuncture.
Assures that proper specimens are obtained, and physiological properties and test requirements are maintained until actual testing procedure.
Informs patients on specimen collection procedures to be performed.
Reviews laboratory request orders to ensure all required information is present.
Determines types of specimens to be collected and if proper patient preparation has been accomplished.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Graduate of a school of practical or vocational nursing approved by the appropriate State [ ] agency and/or [accredited by] the National League for Nursing [Accrediting Commission (NLNAC)] at the time the program was completed by the applicant. Verification can be obtained from the State approving agency [or] the National League for Nursing [Accrediting Commission], 61 Broadway, 33rd Floor, New York, NY 10006. [Additional information may also be obtained from the NLNAC Website. Exceptions include:
Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions.

Licensure: Full, active, current and unrestricted licensure as a [ ] licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN[/LVN] who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16
Physical Standards: According to VA Directive and Handbook 5019.
English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Grade Determinations: In addition to the basic requirements stated in section A, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
For the GS-03 level, experience and education: None beyond the basic requirements.
For the GS-04 level, experience: Six months of qualifying experience as an LPN or LVN OR
Experience and Education: Graduation from an approved school and one year of experience that involved nursing care work in a hospital outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures. OR
Education: Graduation from an approved school of at least 24 months duration. LPNs at the GS-4 grade level perform all duties expected of employees at the GS-3 grade level; however, they are expected to exercise greater judgment, require less supervision, and to operate in accordance with all established policies, procedure, and techniques.
For the GS-05 level, experience: Completion of at least one year of qualifying experience at the GS-4 level or equivalent and fully meeting all performance requirements for the GS-4 LPN/LVN. The full performance level of this vacancy is GS-6. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-3 to GS-6.
Physical Requirements: Physical requirements include but are not limited to the incumbent may experience long periods of standing, sitting, walking, bending, pushing and pulling. Lifting is required though mechanical lifting equipment and/or other approved patient handling aids are used to prevent the manual lifting and handling of patients/residents except when necessary, such as in a medical emergency. While performing the duties of this job, the employee is regularly required to stand and walk. The work requires long periods of standing and walking. The employee must occasionally lift and/or move up to 50lbs. Patient mental status and physical habitus will require the incumbent to be able to assist patients during transfer that is required of examinations. Specific vision abilities required by this job function include but are not limited are close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
Must be able to pass all requirements according to Optional Form 178 (Certificate of Medical Examination).
