As the Assistant Nurse Manager, incumbent will work in collaboration with the Nurse Manager for the Primary Care and GEM Clinics located at Kerrville VA. Incumbent will assist in the daily nursing oversight for the Clinic to include staffing assignments, clinical issues, patient complaints, etc. The incumbent will function as a supervisor and assume accountability for the clinical operations of the designated clinics.

Duties

As the Assistant Nurse Manager, incumbent will work in collaboration with the Nurse Manager for the Primary Care and GEM Clinics located at Kerrville VA. Incumbent will assist in the daily nursing oversight for the Clinic to include staffing assignments, clinical issues, patient complaints, etc. The incumbent will function as a supervisor and assume accountability for the clinical operations of the designated clinics. This includes the management, supervision and evaluation of care delivered by nursing and other allied health care staff. The incumbent is responsible for contributing to the implementation of evidenced-based nursing practice. In collaboration with the Nurse Manager, the incumbent will lead/delegate group activities to solve problems or accomplish specific tasks. The incumbent will serve as a role model in the delivery of comprehensive patient/family care and utilize problem-solving skills to minimize or eliminate barriers to high-quality patient care delivery. The incumbent will manage and validate staff competencies, organize educational activities, provide coaching/mentoring to improve performance. Manage employee orientation, annual competency assessments, mandatory TMS training, and maintain staff education folders. The incumbent will assist the Nurse Manager with staff's performance evaluations, Individual Development Plans, sick leave reviews, and staff meetings. The incumbent will communicate and monitor staff compliance with Performance Measures, Policies/SOPs/Protocols, Regulatory Compliance Standards, Patient Safety, Quality Management/Process Improvement activities, and Pathway to Excellence. The incumbent will participate in activities to improve SAIL metrics, customer service, and staff satisfaction. May be required to prepare action plans as related to SAIL metrics, Ambulatory Sensitive Conditions, Clinic Utilization, and Disease Management. This position is 60% clinical and 40% administrative. In the clinical role, the incumbent will perform a wide range of clinical/administrative duties and will utilize nursing skills when assessing and triaging unscheduled patients as walk-ins, over the phone or secure messaging. Assures current, comprehensive, and timely documentation is contained in the patient's medical record and any other pertinent records. Uses the nursing process, evidence-based practice, and effective communication to collaborate with healthcare providers, family/caregiver, internal VHA and community based services and disciplines involved in providing care to the patient in developing the patient-driven holistic care plan for life. The RN must have the ability to work within an intra-professional team to assist in the nursing care of complex populations. In primary care, the RN serves as patient care coordinator for veterans assigned to PACT. The RN coordinates care and collaborates with other staff/services to facilitate the best clinical outcomes, quality of life, and a seamless transition between facilities and levels of care. Must be able to participate in all PACT activities, huddles, and training as well as other relative training.
