The employee serves as a Social Work Associate at the William "Bill" Kling VA Outpatient Clinic located in Broward County, Florida.
Duties

Duties include but may not be limited to:
Expedites treatments and implements objectives for the Veteran, providing total social services to selected cases in which goals can be achieved.
Assists Veterans with filling out applications/paperwork, forms (including but not limited to Advance Directives paperwork), gathers information, handles details around case management and care coordination such as VA benefits, Medicaid applications, assisted living facilities and contract community nursing home placement.
Provides routine case management/care coordination, taking full responsibility for execution of tasks in support of psychosocial needs.
Initiates home health services, Community Adult Day Health Care services, etc. and collects information including contact information about the Veteran's significant others are typical tasks.
Follows Medical Center policies and ensure all activities are updated in the Veteran's medical record.
Performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving Veterans.
Interviews Veterans and/or their caregiver face to face or by telephone to assess the need and develop a patient centered plan to meet the identified needs.
Ensures follow-up to ensure continuity of care.
Maintains knowledge of changes in the referral procedures for community services and makes social work staff aware of the new procedures.
Performs other duties as assigned.
Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications
To qualify for this position, applicant(s) must meet all of the following requirements:
English Language Proficiency - In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position.
BASIC EDUCATION REQUIREMENT: A bachelor's or higher degree from an accredited college or university in major fields of study such as: social welfare, sociology, psychology, public administration, philosophy, education, anthropology, history, economics and/or English. NOTE: You must submit a copy of your transcript with your application. Education cannot be credited without documentation.
SPECIALIZED EXPERIENCE: Requires at least two years of experience as a social welfare worker or social work assistant in a voluntary, public, social, or health agency that is equivalent to the next lower grade in the Federal service. At least one year of this experience must have been in a medical setting. Specialized experience is experience in a program of direct social welfare services that required the application of program policy and operation knowledge in providing social welfare services and/or assisting social workers. Examples of specialized experience would typically include providing assistance and service support as prescribed by social workers in conjunction with patients' treatment plans; interviewing patients; obtaining and compiling data; and assisting with local, state or federal benefits and referrals for food, shelter, transportation, etc. (Resume(s) must clearly support the duties and experience required for this position. IMPORTANT: A full year of work is considered to be 35-40 hours of work per week. All experience listed on your resume must include the month and year start/end dates. Physical Requirements: The work involves sitting, standing, and walking and is carried out within the confines of the office/workspace provided.
Work will be done in Ambulatory Care/specialty care clinic locations. Much of the work will be carried out in the integrated team-work space. The employee works in proximity to Veterans and is therefore, occasionally exposed to communicable disease. The employee may also be subjected to hostile behavior during an interview and defensive maneuvers to protect self may be necessary. Employee may be required to drive a government vehicle to other sites of care; community meeting; outreach events or other appropriate functions.
