All staff physicians assigned to Medicine Service will meet basic requirements and hold a degree of doctor of medicine or doctor of osteopathy from a college or university approved by the Administrator; complete an ACGME accredited residency satisfactory to the Administrator, meet the physical standards as described by the Administrator and will be licensed to practice without restrictions in a state or territory of the United States.

Duties

Duties and responsibilities include but are not limited to: Duties: Duties and responsibilities include but are not limited to: Diagnose, treat, and manage patients with acute medical conditions in the inpatient setting according to accepted standards of care. Admit patients to the W.G. (Bill) Hefner Salisbury VA Medical Center in keeping with good clinical practice standards of what would be expected of a Hospitalist physician. Serve as the admitting and/or attending physician or consulting physician for patients admitted to the Med/Surg wards and ICU. Provide cross-coverage to all inpatient units as well as residents of the Community Living Center (CLC) Perform medical consultations on patients when requested by the Surgical Service, Psychiatry Service, and Emergency Department providers. Provide inpatient Internal Medicine services including clinical procedures within your specialty area of expertise. Complete History and Physicals, Internal Medicine Consultations, Daily Progress Notes, Episodic Care Notes, Medication Reconciliation, and Discharge Summaries as required and in a timely fashion. Enter orders as appropriate in the electronic medical record. Will participate in inter-hospital transfers of patients as needed. Complete Shift Handoff Tool on a daily basis prior to end of shift for sign-out purposes. Close supervision and education of internal medicine residents and medical students in all their clinical activities (when applicable). Participation in educational activities related to the internal medicine. Collaboration with other providers, including advanced practice providers and nursing care providers throughout the hospital. Supervision of Physician Assistant's within the Hospitalist department. As a staff physician in the Hospitalist section, you will be encouraged to participate in the development or on-going research, quality improvement, and teaching as it relates to your professional interests. Maintain Internal Medicine board certification. You will be expected to serve on Peer review, and potentially other committees as requested by the Medical Center Director, Chief of Staff, Chief of Medicine or the Hospitalist Chief. You will be expected to be in compliance with the Medical Staff bylaws and will complete all medical records timely through CPRS (Computerized Patient Record System) in accordance with VA Policies and Regulations. All attending physicians are expected to participate in ongoing process improvements within their specific areas of expertise thereby promoting quality, efficiency, patient safety, and timely access to care for our Veteran patients. Active participation and/or attendance at daily discharge planning meetings (bed huddle), grand rounds and Morbidity/Mortality (M&M) educational conferences. All physicians are expected to attend quarterly Medical Staff meetings and monthly Medicine department staff meetings when they are on duty. All physicians are actively engaged in completing quarterly chart reviews and other quality assurance work within the department. Will ensure timely record completion for all patients as outlined in the Department of Medicine. Completes required/assigned TMS modules by deadline. Being available and responsive to staff in order to provide the best care to our patients during the tour of duty. Completes other tasks as delegated by the Medical Center Director, Chief of Staff, Chief of Medicine, or Section Chief. Providing patient care to Veterans in the areas of Internal Medicine and Acute Care Service Line in the form of primary and/or consultative care. Responsible for the provision of medical diagnostic evaluation and initial assessment of patients to include a minimum of focused history, physical examinations, and pertinent laboratory investigations. Responsible for the evaluation of all new admissions with completion of an admission History and Physical, or completion of the short-form admission note as appropriate. Perform, at a minimum, basic procedures expected of any board eligible Internist. This includes lumbar puncture, thoracentesis, and paracentesis. A proficiency in the placement of central venous catheters is highly recommended. Supervise any residents, students, fellows, or teams assigned to their areas of responsibility. This will include (but is not limited to) at least two attending months per academic year which requires board certification.

To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Preferred Experience and Certifications: Nocturnist/Hospitalist All physicians will be board eligible/board certified in Internal Medicine or Family Practice. Hospitalist Board certified in Internal Medicine. 5 or more years of clinical experience since completion of residency training. Experience and/or training in critical care medicine. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: This position requires a pre-employment physical. Requires lifting 15-44 pounds; pushing (approx. 2 hours); reaching above shoulder; use of fingers and both hands; walking and standing from 3-5 hours and kneeling. Ability for rapid mental and muscular coordination simultaneously. Near vision correctable at 13" to 16" to Jaeger 1 to 4. Far visions correctable in one eye to 20/20 and to 20/40 in the other. Must have depth perception and ability to distinguish basic colors and shades of colors. Hearing aid is permitted.

Education

Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:

1. Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
2. Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
3. For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]

NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
