The South Texas Veterans Health Care System is currently recruiting for a Primary Care Registered Nurse-Assistant Nurse Manager position. The primary base location for this position will be located at the Balcones Heights Clinics, San Antonio, TX.
Duties
As the Assistant Nurse Manager, incumbent will work in collaboration with the Nurse Manager for the Primary Care Clinics located at Balcones Heights Gold & Silver. Incumbent will assist in the daily nursing oversight for the Clinic to include staffing assignments, clinical issues, patient complaints, etc. The incumbent will function as a supervisor and assume accountability for the clinical operations of the designated clinics. This includes the management, supervision and evaluation of care delivered by nursing and other allied health care staff. The incumbent is responsible for contributing to the implementation of evidenced-based nursing practice. In collaboration with the Nurse Manager, the incumbent will lead/delegate group activities to solve problems or accomplish specific tasks. The incumbent will serve as a role model in the delivery of comprehensive patient/family care and utilize problem-solving skills to minimize or eliminate barriers to high-quality patient care delivery. The incumbent will manage and validate staff competencies, organize educational activities, provide coaching/mentoring to improve performance. Manage employee orientation, annual competency assessments, mandatory TMS training, and maintain staff education folders. The incumbent will assist the Nurse Manager with staff's performance evaluations, Individual Development Plans, sick leave reviews, and staff meetings. The incumbent will communicate and monitor staff compliance with Performance Measures, Policies/SOPs/Protocols, Regulatory Compliance Standards, Patient Safety, Quality Management/Process Improvement activities, and Pathway to Excellence. The incumbent will participate in activities to improve SAIL metrics, customer service, and staff satisfaction. May be required to prepare action plans as related to SAIL metrics, Ambulatory Sensitive Conditions, Clinic Utilization, and Disease Management. This position is 60% clinical and 40% administrative.
In the clinical role, the incumbent will perform a wide range of clinical/administrative duties and will utilize nursing skills when assessing and triaging unscheduled patients as walk-ins, over the phone or secure messaging. Assures current, comprehensive, and timely documentation is contained in the patient's medical record and any other pertinent records. Uses the nursing process, evidence-based practice, and effective communication to collaborate with healthcare providers, family/caregiver, internal VHA and community based services and disciplines involved in providing care to the patient in developing the patient-driven holistic care plan for life. The RN must have the ability to work within an intra-professional team to assist in the nursing care of complex populations. In primary care, the RN serves as patient care coordinator for veterans assigned to PACT. The RN coordinates care and collaborates with other staff/services to facilitate the best clinical outcomes, quality of life, and a seamless transition between facilities and levels of care. Must be able to participate in all PACT activities, huddles, and training as well as other relative training.
The incumbent should possess excellent communication and leadership skills, be proficient in database management applications, such as Word, Outlook, Excel, Electronic Health Record, Web-based applications, etc. May be required to fill in for/assist other nurse managers within the Primary/Specialty Care Service line as needed.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Minimum of five years recent nursing experience
Minimum of 3 years' experience in leadership/Management
Minimum of 3 years' recent experience in Ambulatory Care

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.
