Duties

HUD-VASH is an intensive case management program for chronically homeless Veterans aimed at providing psychosocial services to stabilize Veterans residing in housing through HUD's Section 8 voucher system. The HUD-VASH Program Coordinator serves to facilitate implementation of housing best practices strategies at the local level through education, monitoring and coordinating activities. The HUD-VASH Program Coordinator has a wide array of administrative and clinical programmatic responsibilities. They provides analysis, evaluation, and quarterly reconciliation of program data from Public Housing Authorities, HOMES, and local HUD-VASH Program administrative database to ensure compliance with documentation standards and program requirements set by VISN, VACO and HUD. The incumbent oversees program operations and evaluations, gathers and assesses relevant data, develops and implements ideas for improvement, and evaluates the efficacy of improvement efforts. They assist other HUD-VASH Leadership staff in overseeing the revision of the local HUD-VASH Handbook and formulate and implement protocols and policies for the program on an on-going basis in order to meet the ever-changing needs of the program's target population. Additionally, they coordinate program readiness for accreditation reviews. The HUD-VASH Program Coordinator provides leadership, orientation, mentorship, consultation to colleagues, and education for assigned staff. They also coordinate case assignments to staff and provide direct case management. The incumbent functions as an advisor to the HUD-VASH Program Manager and the Assistant HUD-VASH Program Manager and staff concerning Housing Services Best Practices at the individual patient level and the HUD-VASH program. This position requires an advanced level of knowledge and understanding of the homelessness and housing services and benefits available from community, local, State and Federal agencies. The HUD-VASH Program Coordinator must have skills in diplomacy, patient advocacy, and negotiation in spanning boundaries of multiple organizations serving individuals and families experiencing homelessness, such as VA, local public housing authorities, HUD, community homeless services providers, shelters, and transitional housing programs. The position requires knowledge of homelessness and its impact upon Veteran, institutional, and community perceptions and behavior, as well as skill in assisting Veterans to adopt behaviors adaptive to community living. The incumbent is licensed in a state at the advanced practice level. The incumbent in this position functions as the Medical Center's Case Manager for VAMC's Healthcare for Homeless Veterans Program Department of Housing and Urban Development /VA Supportive Housing (HUD-VASH). The incumbent is responsible for monitoring all phases of programming and coordination of case management services for Veterans receiving housing provided through HUD Section 8 housing vouchers. While functioning as an independent professional, the individual is under the direct supervision of the Social Work Supervisor through the Chief of Social Work Service and receives clinical, administrative guidance/mentoring as it relates to Social Work practice. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS -12- Social Worker - Program Coordinator Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Preferred Experience: 3-5 Years of experience working with Mental Illness and Substance Abuse. 3-5 years providing case management and care coordination for persons with mental illness, substance abuse and/or homelessness. 2 years advance clinical practice working with evidenced based risk assessments and interventions. 3 years experience working in crisis services related to suicide, violence and/or abuse/neglect References: VA Handbook 5005/120 Part II Appendix G39, September 10, 2019 The full performance level of this vacancy is GS-12. Physical Requirements: The work is sedentary. Some work may require movement between offices, hospitals, warehouses, and similar areas for meetings and to conduct work. Work may also require walking/standing, in conjunctions with travel to and attendance at meetings and/or conferences away from the work site. Incumbent may carry and lift light items weighing less than 15 pounds.
