This position is located at the VA San Diego Healthcare System in the Social Work Service area, with remote supervision provided by Physical Medicine and Rehabilitation OT Supervisor. The primary purpose of the work is providing occupational therapy services to homeless Veterans as part of the VASH interdisciplinary team to maximize function and independence/safety.

Duties

Functions or Scope of Assigned Duties: Intervene to maximize safety and function in the home and community for Veterans that are in the process of being housed or are housed through the HUD-VASH program in the San Diego VA catchment area. Assess and address the Veterans' strengths and limitations related to their ability to live safely and effectively in the community and engage in meaningful activities. Prescribe and train Veteran and caregivers in the use of durable medical equipment (DME). Recommend changes in the home environment, level of supervision required, if any and arranging the living space so it is safe and functional; managing time, medication, meals and finances, attaining and maintaining employment or volunteer positions, parenting, and maintaining medical appointments. Provide caregiver training. Make recommendations for referrals to other services as needed. Plan, develop, oversee OT VASH clinical activities, consulting the Social Work Supervisor in matters of delegation of resources, conflicting regulations or unusual or unprecedented circumstances. Advise the OT Supervisor and other therapists on regulations governing home care. Serve as subject matter expert in all areas above. The location and provision of the therapist's work is in VASH is the community in general, and VA facilities. Documents patients progress in accordance with established policies. Attends and actively participates in conferences, rounds, etc. as part of an inter-disciplinary team. Maintains clinic area and equipment in a safe and orderly manner in accordance with safety and infection control policies, JCAHO/CARF regulations and other relevant regulatory policies. Meets all mandatory VASDHS training requirements. Adheres to patient safety standards and infection control methods. Ensures all clinical activities are consistent with OT scope of practice. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education and/or Experience. The individual must meet at least one of the following requirements below: Bachelor's degree in occupational therapy and two (2) years of experience as an occupational therapist; NOTE: The baccalaureate degree must be from an approved program prior to the AOTA January 1, 2005 decision that the Accreditation Council for Occupational Therapy Education (ACOTE) would only accredit master or doctoral degree programs in occupational therapy. OR Bachelor's degree in occupational therapy and two (2) full years of graduate education in a related field. NOTE: The baccalaureate degree must be from an approved program prior to the AOTA January 1, 2005 decision that ACOTE would only accredit master or doctoral degree programs in occupational therapy. OR Master's Degree or higher in occupational therapy. Individuals must be a graduate of a degree program in occupational therapy approved by the ACOTE or predecessor organizations. This is inclusive of an internship (supervised fieldwork experience required by the educational institution). ACOTE is the only accreditation agency recognized by the United States Department of Education and the Council for Higher Education Accreditation. Graduates of foreign occupational therapy programs meet the requirements of subparagraph [3b(2)] if they have a current, full, active and unrestricted license referred to in subparagraph [3e] of this appendix. Certification. Candidates must possess a current NBCOT certification as an OT. State Licensure. Candidates must possess a full, current, and unrestricted state license, to practice occupational therapy in a state, territory or Commonwealth of the United States (i.e., Puerto Rico), or in the District of Columbia. Exceptions for the Graduate Occupational Therapist. OT graduates from an approved occupational therapy program who otherwise meet the minimum qualification requirements, but who do not possess NBCOT certification and/or state licensure, may be appointed, pending certification and/or licensure, as a graduate OT on a full-time temporary appointment not-to exceed two years under the authority of 38 U.S.C. § 7405(c)(2). Graduate OTs may only be appointed at the GS-9 grade level and may not be promoted/converted to the GS-11 level until licensure and/or certification is obtained. For grades levels at or above the developmental GS-11 grade level, the OT must be certified and licensed. A graduate OT may provide care only under the direct supervision of a licensed OT who meets all state regulatory requirements. Temporary graduate OT appointments may not be extended beyond two years, or converted to a new temporary appointment. Grandfathering Provision. All persons employed in VHA in this occupational series or in another occupational series that are also performing the duties as described in the qualification standard on the effective date of this qualification standard are considered to have met all qualification requirements for the grade held including positive education and licensure/certification/registration that are part of the basic requirements of this occupation. Employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: They may be reassigned, promoted up to and including the full performance level, or changed to lower grade within the occupation, but may not be promoted beyond the full performance level or placed in supervisory or managerial positions. If an OT who was retained under this provision leaves the occupation, the employee will lose protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. OTs initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all the basic requirements of this qualification standard, must maintain the required credentials as a condition of employment in the occupation. OTs who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Occupational Therapist, GS-12 Education/Experience. Completion of one year of experience equivalent to at the GS-11 grade level and directly related to the position being filled. Demonstrated KSAs. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of occupational therapy principles and techniques consistent with current clinical standards based on OT theory and evidence based practice. Knowledge is inclusive of physical, occupational, cognitive, and psychosocial functional deficits. Ability to collaborate and communicate orally and in writing with all internal and external stakeholders. Ability to use critical analysis, clinical reasoning, and creativity to independently solve complex problems related to adapting and modifying assessments, treatment plans, activities and procedures to meet the needs of patients. Skill in procuring, fabricating, adjusting, adapting, and modifying orthoses, splints, and adaptive equipment for activities of daily living (inclusive of durable medical equipment). Ability to conduct OT related in-service and clinical training. Preferred Experience: Minimum of 2 Years of Experience as an Occupational Therapist is required. This position will be working with the homeless population, experience in mental health is preferred. Driving a vehicle throughout San Diego County; and Working with a high degree of independence. The Occupational Therapist hired to work in this position will need to be able to assess the home of the patient (patients are housed throughout the San Diego County catchment area). The work location for this Occupational Therapist position is in the community as well as at VA facilities.
