The Care Coordination & Integrated Case Management (CC&ICM) Social Work Service Co-Champion Coordinator is a Supervisory position in Social Work services at the VA Boston Healthcare System located in Brockton, MA. This position is a Full-time at 40 hours per week.

Duties

The Care Coordination & Integrated Case Management (CC&ICM) Social Work Service Co-Champion, hereafter referred to as the Incumbent, is a Social Work Program Coordinator whose primary responsibility is to oversee the implementation and daily operations of CC&ICM. The Incumbent serves as the VA Boston Healthcare Systems' Care Coordination & Integrated Case Management (CC&ICM) Social Work co-champion and point of contact for the CC&ICM program and planned expansion. The Incumbent has expertise in coordinating care and serves as the integration liaison between facility leadership and CM Staff. Implementation will be highly complex, involving collaboration and planning across multiple services and disciplines. The GS-12 Social Work Program Coordinator performs duties independently, reporting to the Associate Chief of Social Work Service or designee. Supervision/consultation is of an administrative and professional nature; with the supervisor providing guidelines as to the general policy and direction of Social Work and the CC&ICM the incumbent coordinates. The Incumbent must communicate effectively with team members, facility treatment teams, and community agency staff to facilitate continuity of care for Veterans in the community while enhancing the working relationship between the VA and community agencies. The Incumbent must build and sustain positive relationships based on trust and respect, collaborate well with others, show sensitivity and compassion for others, handle conflict and negotiations effectively, and remain competent in high-pressure situations. The Incumbent must respond appropriately to new or changing conditions, handle multiple inputs and tasks responsibly, and accommodate new situations and realities. Professional judgment and expertise will be used to make decisions about services and treatment provided to Veterans and their significant others/family members are made independently. However, in programs with multiple team members, the decisions are made collaboratively. The Incumbent will work with colleagues assigned to the program in the assessment, development, implementation, and evaluation of strategies and protocols in support of program operations and clinical care. The Incumbent is given the authority to make their own work assignments within the program. The Incumbent is professionally accountable for the quality and quantity of the work. The Incumbent consults with the Chief of Social Work Service or designee and team members around complex or unusual situations but is ultimately responsible for decisions and actions that are taken. Quality of work is monitored through audits, VAMHCS Social Work peer review, and other evaluations as required by Social Worker Qualification Standard GS- 185 Veterans Health Administration. Work Schedule: Full-Time; Monday - Friday; 7:30 a.m. - 4:00 p.m. Compressed: Not Authorized Telework: Authorized. Telework can be performed on a less than full-time basis anywhere other than a VA facility or using VA-leased space (example: home). Veterans' preference does not apply for internal or other current permanent Federal agency employees. Current Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held. This is not a Bargaining Unit position. This position is covered by locality-based comparability pay. Selectee will be required to travel to other VA campuses, CBOC's and the community. Narrative responses to the knowledge, skills and abilities (KSAs) will be required from the selectee in order to proceed with the appointment. Incomplete applications will not be considered.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/17/2023. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j. Grade Determinations/Minimum Qualifications for GS-12 (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Ability to independently organize work, set priorities, and meet multiple deadlines. (b) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. (c) Ability to ensure provision of clinical social work services. (d) Ability to provide staff training and development. (e) Skill in interpersonal relationships in dealing with employees, team leaders, and managers. Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies.
