About the Position: This position is located at the Utilization Management Department at Naval Health Clinic (Pearl Harbor), which is on the Island of Oahu, Hawaii.
This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD Workforce to recruit and appoint qualified candidates to positions in the competitive service.
Advance in Hire may be authorized. This is only available for those candidates who are new to Federal service.
Duties
Identifies patient and disease specific education and care needs, to provide comprehensive, patient-centered, proactive strategies promoting prevention, health, and wellness concepts that support the provider' s plan of care.
Addresses chronic conditions such as: asthma, chronic obstructive pulmonary disease, diabetes, dyslipidemia, heart failure, and hypertension.
Evaluates the patient's response to the plan of care and provides documentation in the medical record that the plan and quality of the services offered to the patient correspond to their identified needs.
Provides monthly reports to the primary health care teams regarding their performance on chronic disease population management and other key disease management metrics.
Provides one-on-one and group patient education appointments and evidence-based practice on relevant care-related items, including but not limited to disease processes, medications, labs, basic nutrition and exercise, and health promotion.
Requirements
- Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
- This position may require travel up to 10% of the time.
- Must be able to obtain and maintain a Secret security clearance.
- Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series.
- This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination.
- This position requires the completion of a pre-employment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health and ability or fitness to perform the duties of the position.
- Position may be require heavy lifting of up to 44lbs.
- This position is a Testing Designated Position subject to applicant testing and random drug testing.
- Must obtain and maintain a current Basic Life Support (BLS) training and certification from the American Heart Association or Military Training Network.
- Employee must obtain and maintain a favorable result from a Criminal History Background Check (CHBC). This position requires at least one contact (i.e. patient encounter) per week or at least 4 contacts per month with children under the age of 18.

Basic Requirement for Nurse (Disease Management):
Degree: A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.).
Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Foreign Degree: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status. Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. Active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Education and Experience: Diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) which includes: providing comprehensive disease management services; assesses and identifies individual patient care needs; and educates patients (Note: You must attach a copy of your transcripts.)
OR
Education: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field including but not limited to Nursing, Advanced Practice Nurse, etc.

FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
