This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The RN Oncology Nurse Navigator (ONN) dedicated responsibility is the coordination of the cancer patients' journey through the care system, at times across disciplines and care settings. The ONN is a professional nurse with oncology-specific clinic knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers.
The major duties and responsibilities include, but are not limited to:
Coordinates medical oncology and multidisciplinary staff in the delivery of complex outpatient care including evaluating patients, performing or arranging for diagnostic and therapeutic procedures, triaging to appropriate care settings, scheduling patients for admissions, and gathering baseline and follow up data.
Provides expertise in specialty area to address educational, clinical, and system needs and issues in order to promote quality care for the veteran cancer population across a continuum.
Provides appropriate and timely education and teaching to patients, family, and caregivers as needed, in order to facilitate understanding and support of diagnosis, treatment options, side effect management, and post-treatment care and survivorship. Reinforces adherence regarding treatment schedules, protocols, and follow up.
Serves as consultant to multidisciplinary staff in initiating and implementing problem solving strategies for patients with complex care plans. Develops and utilizes practice guidelines or clinical pathways as necessary to streamline multi-disciplinary evaluation and treatment for cancer patients. Functions as a stable point of contact for the cancer patient at any stage in the cancer process, advocating for patients including shared decision making and patient-centered goals of care with optimal outcomes.
Makes appropriate assessments and referrals as needed to meet the unique needs of this specialized patient population.
Facilitates consultations, participates in the care of patients seen in oncology clinics and provides follow up care. Participates in the Cancer Care Committee, Tumor Boards, and has a major role in the multidisciplinary patient conferences.
Collaborates with Oncology Nurse Practitioner and Cancer Care Committee members in maintaining and ensuring the requirements for the survivorship care program and American College of Surgeons (ACoS) Commission on Cancer program are being met for ongoing accreditation.
Collaborates with the Medical Oncology multidisciplinary staff, physicians, nurses, psychologists, social workers, pharmacists, dieticians, chaplains, and cancer program staff to ensure that the multidisciplinary needs of the cancer patient are being met.
Assists the Oncology physicians and nurse practitioners in Stanford Radiation Oncology consult coordination, working with community care team to ensure timely appointments and assist with obtaining treatment summaries and putting them into CPRS.
Collaborates with the Oncology providers initiating (as well as continuous ongoing) Life Sustaining Treatment goals of care discussions with patients, families, and caregivers that look to improve and support effective quality of care for our patients and improve our SMR30 (Standardized Mortality Ratio) metric for this specific population.
Provides direct administrative support to the Outpatient Oncology Nurse Practitioners and Oncology Physicians with tasks (not limited to) responding to patient phone calls and MyHealtheVet messages, and tracking and collecting of patient data and information for diagnostic and treatment purposes.
Ensures new patients have appropriate workup including diagnostic tests, signed consent, and works cooperatively with Primary Care and other specialties to arrange scheduling in the Hematology/Oncology Infusion clinics.
Participate in quality improvement activities, including the monitoring of treatment guidelines, metrics, and outcomes.
Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience:
>5 years nursing experience
Oncology experience (preferably at least 2 years)
OCN Certification or ONS Chemotherapy/Immunotherapy Provider Card
Case management experience
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

Physical Requirements: This position requires heavy lifting, 45 pounds heavy carrying, 45 pounds straight pulling (2 hours); reaching above shoulder; use of fingers; both hands required; walking & standing 8 hours - intermittently; repeated bending; both legs required; ability for rapid mental& muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing (aide permitted). Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
