The Traveling Veteran Coordinator (TVC) Nurse is a licensed professional Registered Nurse (RN) assigned to coordinate necessary or ongoing outpatient care for Veterans traveling and address episodic care while traveling. This includes the usage, utilization and co-management of Primary Care Management Module (PCMM). The TVC RN follows national directives to ensure compliance with VHA Directive 2012-016.

Duties

Oversee TVC/PCMM Programs and coordinates necessary or ongoing outpatient care for Veterans traveling out of their preferred facility area. Use advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes. Provides Patient-Centered coordination of care for the traveling veterans. Make recommendations for referrals to appropriate specialty care and may recommend PACT/dual enrollment based on complexity and need of the veteran. Responsible for communication with the TVC at the non-preferred facility to obtain/send information. Attend all TVC meetings: local, VISN and National. TVC is responsible to ensure program is functioning per VISN and National guidance, ensuring to comply with documentation requirements. Ensure traveling veteran's care is transitioned appropriately back to the preferred facility PACT team or specialist for ongoing care needs when the Veteran returns to his or her preferred facility. When non-routine follow-up is required at the preferred facility (e.g., post-discharge follow-up, continuation of complex treatment, medical evaluation in progress, new diagnosis or treatment), transfer of care between facilities will occur with the assistance of the TVCs, so that care arrangements can be made with the appropriate specialist or PACT provider at the preferred facility. The referring facility provider's name and contact information will be provided to the involved TVCs (and included in a CPRS progress note) so as to facilitate provider-to-provider contact when needed. Lead and organizes delivery of care to assure continuity of care and peer accountability for practice, including but not limited to access to care. Coordinate the Non-VA Care Referral review process for traveling Veterans who require health care services that are not available at the non-preferred facility in accordance with Non-VA Care Coordination (NVCC) Standard Operating Procedures established by the VHA Chief Business Office (CBO). Guide, develop, and support staff from a leadership perspective. Use professional standards of care, scientific evidence, and practice to evaluate programs/service. Demonstrate grade-appropriate and consistent critical thinking skills: The nurse can recognize the patient has a problem. he nurse can manage the problem safely and effectively, recognizing his/her scope of practice. The nurse has a relative sense of urgency. The nurse takes the right action for the right reason. Treat all patients and families with dignity and respect. Listen to and honors patient and family perspectives and choices. Patient and family knowledge, values, beliefs, and cultural backgrounds are incorporated into the planning and delivery of care in a non-judgmental, non-discriminatory manner. Maintain professional boundaries with patients, families, and employees. Behavior as a role model is transparent and above reproach. Initiate/participate in consultations to health care teams regarding ethical issues. Identify potential and actual problems involving resources and/or safety. Takes action to avert or manage the situation in a manner which meets the needs of patients in a safe/therapeutic manner. Establish patient care priorities based on patient needs that ensures patient care can be delivered in a safe, efficient, and cost-effective manner. Consistently demonstrate effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers. Collaborate with patients, families, health care practitioners, and health care leaders in policy and program development, implementation, and evaluation, as well as in the delivery of care. Initiate and applies a collaborative team approach in identifying, analyzing, and resolving problems at the service or medical center level. Other duties as assigned. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Traveling Veteran Coordinator experience - at least 1 year of experience Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves considerable standing, walking, stooping, bending, reaching, lifting and turning. It requires repositioning patients and assisting with transferring patients to wheelchairs/stretchers. Mechanical devices when available are required to be used for heavy or difficult to lift patients. The work may involve pushing or lifting weight in excess of 50 pounds and going up and down flights of stairs. The position requires visual and auditory acuity at a level to provide safe and effective nursing care. The positions requires the nurse to work for periods of time with arms above shoulder level or with the neck in a fully flexed or extended position and the ability to physically control or protect him/herself from physically abusive patients.
