This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The incumbent will serve as a HUD-VASH Geriatrics Specialist for VISN 21 in the Northern California Healthcare System (NCHCS) Social Work Service Chief office. This is a collaborative position that works closely with both the HUD-VASH and GEC National Program Offices. The incumbent has administrative responsibility for the development of novel collaborations designed to meet the clinical and housing needs of aging and disabled homeless and/or formerly homeless Veterans. Additionally, the incumbent is accountable for clinical program effectiveness and modification of service patterns to aging homeless and/or formerly homeless Veterans with complex and chronic disabling disorders, including Veterans in advanced stages of chronic disease or nearing end of life. Assignments may include clinical settings where they have limited access to onsite supervision such as CBOCs or satellite outpatient clinics. The incumbent has knowledge of screening, assessment, diagnosis, evidenced-based treatment recommendations, and systems of care for aging and/or disabled, homeless Veterans. Major duties include, but are not limited to: Participate in the development, evaluation, and communication of standards, policies, procedures, and plans of care for partnerships serving aging and disabled HUD-VASH Veterans, including partnerships between GEC and HUD-VASH programs. Educate VISN and Medical Center level staff in Homeless Programs regarding various models for GEC and HUD-VASH partnerships. Evaluate GEC and HUD-VASH partnerships using relevant VA statutes and regulations and take action to improve identified deficiencies. Track GEC and HUD-VASH partnership program development, statistics, data management, identify significant findings, problem solve including identifying barriers to implementation, and complete reports pertaining to development of these programs. Prepare reports and statistics for facility, VISN, and national use. Identify and develop program resources in the community and across VAMCs, providing liaison with these providers. Assist with needs assessments across the VISN to support in program development identification. Serve as liaison between Homeless programs, GEC programs, and other community programs to develop programming and housing designed to serve the aging and/or disabled homeless Veteran population. Developing and sharing resources related to VA and community social service programs to support GEC and HUD-VASH program development. Partner with Public Housing Authorities, the Office of Asset and Enterprise Management, and community partners to develop project-based HUD-VASH housing and other appropriate housing opportunities for aging and disabled HUDVASH Veterans. Establish and maintain positive working relationships with internal and external customers. Work Schedule: Varying tours available with exact schedule to be discussed during the interview process. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations:Social Worker (Program Coordinator), GS-13 Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs.(a) Skill in coordinating and implementing one or more specialty program(s), across the facility.(b) Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.(c) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).(d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.(e) Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.(f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.(g) Knowledge of organizational structure and impact on program operations.
