This Home Care Coordination Social Worker position is in the Geriatrics and Extended Care services, at the VA Boston Healthcare System. This position is located at the Brockton, MA location. This position is full-time at 40 hours per week.
Duties
Individuals assigned as GS-12 Social Workers are at the full performance level. Incumbent is a professional social worker assigned to the Home Care Coordination (HCC) Program as a member of the interdisciplinary treatment team. The incumbent is responsible for clinical Social Work practice in the area or program to which assigned, including promotion of Social Work services as a key component of treatment. The incumbent directs/coordinates clinical and psychosocial services and is accountable for the overall effectiveness of the services provided, modifying services to best meet the treatment needs of veterans and to promote efficient practice. The social worker will provide services both via telephone contact and in the homes of veterans enrolled in HCC.
Social Worker Duties and responsibilities:
The incumbent: Supports the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility.
Serves on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Associate Chief of Staff, Geriatric and Extended Care Services. Keeps their supervisor apprised of problems and recommended solutions to problems encountered in the incumbent's area of responsibility.
Furthers their own professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment.
Evaluates his/her practice on an on-going basis through participation in professional peer review, case conferences, research studies, or other organized means. Is active in and accepts responsibility for the development and maintenance of professional standards of treatment.
Assists with the orientation and learning of less experienced social workers and, if interested, serves as a preceptor for social work students.
Demonstrates knowledge and skill in the use of software applications for drafting documents and data management as well as other computer systems in use by the VHA.
Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Joint Commission standards, and facility standards.
Work Schedule: Monday-Friday, 8:00am-4:30pm

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements: The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in
VHA, all applicants must meet the following:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
Grade Determinations:In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
Grade level requirements at the GS-12:
Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
In addition to the experience above, candidates must demonstrate all the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
