This position is located within Social Work Service at the Veterans Affairs (VA) Medical Center in Houston, TX. The Social Work Associate assists in the administrative and clinical operation of the Social Work Service Line.
Duties

The position requires engagement in five major components of Social Work Service:
Case Management - Identifies case management needs. The intake assessment evaluates the range of the veteran's health care and resource issues. In coordination with the Social Work and Nurse Case Managers, identifies those veterans who can benefit from case management services. As part of this function, serves as the primary staff person in the program's case management consult process. Receives all external and internal consults for Social Work Service case management.
Staff Education - Completes initial case management screens on veterans who are new to the VA. Participates in the program's process improvement initiatives. This includes presenting proposals, refining processes and engaging in implementation of process improvement initiatives.
Program Evaluation - Complies with agency directives, circulars, procedures and policies. Maintains records and statistics, completes reports, prepares correspondence, evaluates program activities, and carries out special project assignments as required. Provides consultations to other professional hospital staff.
Performs other related duties as assigned.
Work Schedule: Monday - Friday, 8:00 a.m. - 4:30 p.m.

Qualifications
To qualify for this position, applicant(s) must possess all of the following qualifications:
BASIC EDUCATION REQUIREMENT: Bachelor's degree (or higher) from an accredited college or university in major fields of study such as: social welfare, sociology, psychology, public administration, philosophy, education, anthropology, history, economics and/or English. NOTE: You must submit a copy of your transcript with your application. Education cannot be credited without documentation.
AND
SPECIALIZED EXPERIENCE: Two years of experience as a social welfare worker or social work assistant in a voluntary, public, social, or health agency. One year of this experience must have been in a medical setting and equivalent to the next lower grade (GS-7) in the Federal service. Examples of qualifying experience include: providing assistance and service support as prescribed by social workers in conjunction with patients' treatment plans; coordinating outreach events; interviewing patients; assisting with enrollment and intake assessments; and identifying case management needs. NOTE: Your experience must be fully documented in your resume and must include job title, duties, month and year start/end dates AND hours worked per week.

ENGLISH LANGUAGE PROFICIENCY: In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position.
