Duties
The Social Worker (Program Coordinator) is an advanced practice level clinical social worker assigned to the Mental Health Service at the Oscar G. Johnson VA Medical Center (OGJVAMC) serving as a member of the Homeless Program team. This position includes interfacing with a variety of VA and community-based programs such as local shelters, Continuums of Care, and the Contract Residential Services (CRS) program. As a professional staff member of Mental Health Service, the incumbent participates in goal setting and treatment service planning discussions; participates in professional peer review of his/her own work and that of other staff; and maintains and upgrades professional knowledge and skills by involvement in appropriate continuing education activities.
Major Duties:
Initiates, coordinates and participates in program site inspections utilizing the Health Care for Homeless Veterans (HCHV) Program Handbook, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices.
Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies.
Monitors the clinical care and assesses the compliance of the programs receiving funds.
Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless veterans as well as provides current information on services and benefits available in the VA for HCHV Program participants.
Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
Provides intensive case management services to assigned veterans and significant others with assertive outreach, individualized treatment planning, multidimensional orientation, and intensive delivery of services.
Completes clinical documentation according to all required standards.
Travels and/or provides services to community clinics or outposts (e.g., Community Based Outpatient Clinics) as requested.

Qualifications
Basic Requirements:
U.S. citizenship: Must a citizen of the United States. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
Education: Must have a Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED)
Licensure. Must be licensed or certified by a state to independently practice social work at the advanced practice level to qualify at the GS-12 level. (COPY OF LICENSE REQUIRED)
GS-12 GRADE REQUIREMENTS:
Experience: Must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget.
Applicants must also meet the Demonstrated Knowledge, Skills, and Abilities listed below:
1. In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
2. Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring.
3. Ability to supervise multidisciplinary staff assigned to the program.
4. Ability to organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s).
5. Ability to provide training, orientation, consultation and guidance within clinical specialization of practice.

The full performance level of this vacancy is GS-12.
Physical Requirements: The work is sedentary, but requires some walking, standing, bending, the carrying of light items such as books and papers, and traveling.
