The Patient Safety Manager (PSM) is a registered professional nurse, qualified by education and experience to provide clinical and administrative oversight to the quality of patient care and the appropriateness and timeliness of services provided. The PSM leads improvements in the safety and quality of Veterans' health care across all VA SORCC programs and facilities by implementing the VHA National Center for Patient Safety's(NCPS) systems focused processes to prevent patient harm.

Duties

Duties may include but are not limited to the following: Initiates plans to effectively develop, improve, and sustain patient care safety and quality by integrating critical thinking, clinical reasoning, and formal evaluation skills with knowledge, skills, experiences and competencies related to acute/residential/ambulatory clinical care and healthcare delivery systems. Use statistical and research principles, accreditation/organizational guidance, evidence-based practice, best practice recommendations, and performance improvement strategies and processes in patient safety plans. Advocates for, leads, and manages organizational change to improve the safety and quality of patient care. Competently executes position responsibilities in a manner that demonstrates substantial, continuous, and I CARE values-based leadership, responsibility, and accountability for oversight of population groups and multidisciplinary healthcare programs. Collaborates with internal and external stakeholders to promote ongoing development of a just culture and "culture of safety" within VA SORCC. As the patient safety content expert, provides consultation and support to all levels of facility leadership, committees, and other stakeholders who are directly and indirectly responsible for patient safety, VA SORCC Patient Safety Program (PSP) success, and positive Veteran outcomes. Ensures that senior leaders receive complete, correct, and timely information about actual and potential risks to patient safety and PSP activities and promotes their engagement in decisions, improvements, and approval processes for program-related products and activities, as appropriate. Completes accreditation and inspection-related administrative processes and applications for the PSP. Meets goals through efficient, effective utilization and management of organizational and personal resources. Investigation, evaluation, and monitoring of reports about adverse and close call patient events to identify underlying causes and implement changes needed to reduce the likelihood of recurrence. Organize and lead mandated, formal PSP activities including incident report evaluations, aggregate reviews, and Root Cause Analyses (RCA). Initiate informal, ad hoc, focused improvement actions as appropriate. Completes NCPS data/knowledge management, documentation, and reporting activities to meet time sensitive accreditation, continual readiness, organizational, and VHA NCPS requirements. Organizes the facility's patient safety related knowledge management and quality assurance activities to support safe, high quality patient care and organizational initiatives. Facilitates the development, implementation, and sustainment of systems-based, proactive risk assessment and Root Cause Analysis (RCAs) to protect patient safety and meet related accreditation standards and other regulatory and Veterans Affairs (VA) and VHA requirements. Effectively disseminates patient safety alerts and lessons learned about effective VHA system modifications. Promotes employee behaviors designed to prevent adverse events. Oversees stakeholder implementation and evaluation of The Joint Commission National Patient Safety Goals. Supports program and service leader evaluations of the safety of their patient care processes and compliance with applicable regulatory and accreditation standards by providing patient safety content expertise. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: One to Two years of Patient Safety Manager RN Experience. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Light lifting, under 15 pounds; Use of Fingers; Both hands required; Walking (2-6 hours); Standing (2-6 hours); Climbing, legs only (.5 hours); Ability for rapid mental and muscular coordination simultaneously; Ability to distinguish basic colors; Ability to distinguish shades of colors; Hearing (aid may be permitted); Sitting for 2-6 hours; Typing for 2-4 hours. Environmental Factors: Outside and inside; Excessive heat; Excessive cold; Slippery or uneven walking surfaces; Working closely with others; Working alone.
