This Medical Instrument Technician (Echocardiography) position is in Veterans Health Administration services at the Boston VA Medical Center, located at the West Roxbury location. This position is full-time at 40 hours per week.
Duties
Perform echocardiography exams which range from the routine to the complex in all echocardiography modalities.
Varied administrative, staff development and supervisory functions.
Carrying out complex laboratory procedures such as real-time 3D cardiac ultrasounds utilizing on-line and offline computer enhancement technology and 3D reconstruction technology.
Provide training and supervision to students in cutting-edge modalities.
Perform basic echocardiograms as well as complex procedures such as stress exercise and dobutamine echocardiograms, saline contrast studies, contrast echocardiography, research protocols, valve cases, and assist with transesophageal echocardiograms as needed.
Must have knowledge of regulatory requirements for the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and lab accreditation standards (IAC).
Must be able to set up and maintain the echocardiography laboratory's Transesophageal equipment.
Must work with hospital administration and outside JCAHO inspectors to ensure compliance.
Must assist the supervisor in writing lab and scanning procedures, protocols and policies for Quality Assurance and IAC accreditation.
Assist the supervisor in managing and maintaining IAC accreditation for the Echo Lab, heading certain portions of the Lab accreditation as assigned.
Compile QA data and on an ongoing basis, present findings to the Medical and Technical Directors when instructed.
Assist in organizing IAC accreditation quarterly meetings and present required data delegated by the supervisor.
Act as a lab liaison between IAC and the echo lab to ensure all required documents are submitted.
Assist the supervisor and/or technical director in creating ideas to better manage workflow and organization of the laboratory.
Must act as a liaison to triage equipment problems and communicate with the other departments to correct malfunctions in a timely fashion.

Qualifications

Basic Requirements:

English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English.
Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. See the grade requirements part of this standard for information regarding educational substitutions.
Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grandfathering Provision. The following is the standard grandfathering policy for all Title 38 Hybrid qualification standards. Some of these provisions may not apply to this occupation. All persons employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
Such employees in an occupation that does not require a licensure/certification/registration, may be reassigned, promoted, or demoted within the occupation.
Such employees in an occupation that requires a licensure/certification/registration only at higher grade levels must meet the licensure/certification/registration requirement before they can be promoted to those higher grade levels.
Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard.
Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.
If an employee who was retained in an occupation listed in 38 U.S.C. § 7401(3) under this provision leaves that occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. Grade Determinations: At least 1 year of experience comparable to the next lower grade level (GS-8), which is directly related to the duties of the position to be filled. One year of experience includes, but is not limited to: identify and record anomalies indicative of disease, injury, or other medically significant condition from ultrasound imaging and simultaneous recordings of the Doppler. They incorporate the causes and results of a variety of symptoms and conditions into a plan of ultrasonic diagnostic examination. They change and develop sounding techniques to accommodate such variables as limited patient mobility, variation in physical condition or dimension of the patient, presence of prosthesis or foreign objects, and ultrasonic response of different body parts. They produce diagnostic quality images and Doppler frequency recordings using M-mode, two-dimensional, and Doppler ultrasound, etc. Additionally, applicants must fully meet the Knowledge, Skills, and Abilities (KSAs) at that level. This would be experience which demonstrates possession of the knowledge required in order to provide all aspects of echocardiography services.
In addition, the candidate must demonstrate, in their resume, the following Knowledge, Skills & Abilities (KSAs) and the potential to acquire the assignment specific KSAs designated by an asterisk (*):
1. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to Echocardiography.
*2. Knowledge of JCAHO and other regulatory requirements.
3. Ability to communicate effectively and professionally with employees at varying grade levels.
*4. Ability to manage and supervise employees.
*5. Ability to provide, or provide for, staff development and training.
Preferred Experience: 5 years of experience in adult Echocardiography, ability to travel to satellite location, knowledge of stress treadmill and dobutamine stress echo, knowledge of strain imaging, and knowledge of 3D imaging. Basic Cardiac Life Support (BCLS), ARDMS or CCI certification/registry is desirable.

The full performance level of this vacancy is GS-09. The actual grade at which an applicant may be selected for this vacancy is a GS-09.
Physical Requirements: The incumbent must be able to travel to treatment areas with ease while pushing an Echo Machine. The incumbent must be able to stand for up to 8-10 hours while providing patient care. The incumbent must be able to lift up to 20 pounds of equipment, supplies and records. The incumbent must be able to manipulate the probes during the procedure, pushing against the patient's chest in order to obtain the appropriate images. Additionally, the incumbent will be required to transport patients on stretchers, beds and wheelchairs. There is the potential for extended periods of sitting at computer terminals. The incumbent is expected to use proper preventive ergonomic techniques to perform duties.
