The St. Louis VA Health Care System is seeking a Clinical Pharmacy Specialist (Acute/Surgical Care) to act as a mid-level practicioner requiring the highest level of pharmacist clinical privileges with minimal supervision. The Clinical Pharmacy Specialist is responsible for pharmacotherapy management, provision of drug information services, medication consultation, patient/family education, and precepting of pharmacy residents.
Duties
Clinical Functions
Responsible for medication therapy management as an Advanced Practice Provider.
Serve as a consultant to other health care providers for intensive medication therapy management services.
Documentation in the medical record of all relevant findings from drug therapy monitoring and management activities in a timely manner.
Provides patient care that is appropriate to the cognitive, emotional, and chronological maturation needs of the patient.
Educational
Conducts in-service education programs for physicians, nursing, pharmacy staff and others as assigned.
Educates and trains other health care disciplines on the safe, rational, and cost-effective use of medications.
Participates in the training of pharmacy residents and students.
Responsible for maintaining and improving professional competency by keeping up with information on new drugs, disease state management, and innovations in pharmacy practice.
Administrative and Organizational Support
Assist Pharmacy and Surgery Services in accomplishing the mission and goals as related to the provision of pharmaceutical care.
Contribute to medical center committees when assigned to provide thoroughly investigated, unbiased, well-documented drug information.
Support of formulary and medication related initiatives.
Documents workload, interventions, and outcomes using appropriate methods on a periodic basis as required.
Responsible for the operational status of various aspects of pharmacy programs.
Customer Service
Interacts with the internal/external customers in a manner that proactively enhances customer service and satisfaction.
Exhibits courtesy, tact, cooperation, and respect for veterans, their families, visitors, and other staff at all times.
Uses discretion in appropriately safeguarding confidential information.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.
Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Grade Determinations: To be eligible at the GS-13, applicants must possess one full year of experience equivalent to the next lower grade level, GS-12 AND, demonstrate the following knowledge, skills, and abilities (KSAs):
Ability to effectively supervise subordinate staff.
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards pertaining to pharmacy.
Skill in managing people or programs.
Preferred Experience: Successful completion of PGY-1 pharmacy residency training and/or equivalent training and direct patient care experience to support scope of practice. Successful completion of PGY-2 pharmacy residency in applicable specialty. Other preferred experience include Board of Pharmaceutical Specialties certification or other comparable credential.
