This position is assigned to the Mental Health Integrated Community of Care at the Minneapolis VA. Incumbent is the Compact Act Program Coordinator. The Program Coordinator is directly responsible for the provision of clinical and administrative care of Veterans enrolled under the Compact Act and other duties as assigned. They will serve as the liaison between community care, outpatient teams, and inpatient teams to ensure appropriate enrollment and management of veterans in Compact Act care.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Major duties include, but are not limited to: Screening/Assessment: The incumbent will respond to referrals from VA staff, the Minneapolis VA Health Care System, Community Based Outpatient Clinics (CBOCs) and community agencies. The incumbent will complete or arrange for identified care within the Minneapolis VAHCS care system, as well as non-VA MH resources. They will work with VISN Staff, Eligibility, and others assigned to Compact Act to ensure regional collaboration of care and information. The incumbent will lead the clinical review team, and manage, facilitate clinical review to ensure that Veterans are being considered/enrolled in Compact Act according to program directives. Referral to service providers: Throughout the eligibility period the incumbent a subject matter expert on Suicide, VA Health Care System and/or community resources. They will collaborate with other service providers in reassessing the Veteran's needs. The incumbent is responsible for educating the Veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker. Resource Usage/Development: The incumbent is responsible for maintaining, updating, and expanding a current resource file of VA Health Care System programs/services and community social service programs and enhancing the content to the benefit of Veterans. The incumbent will be responsible for developing relevant SOP's and workflow processes for program. They will assist with providing any necessary referrals. Advocacy:The incumbent will act as an advocate when it serves the best interest of the Veteran/family. They will need to advocate for the needs of Veteran/Family based on a collaboration between Federal, State & Community/Private entities that are providing care to Veterans. Advocacy will be needed at all levels to ensure veterans receive high quality, effective and timely access to care and services. When appropriate and feasible, the incumbent will educate and encourage the Veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment. Coordination of non-institutional and institutional services:The incumbent will review the progress notes from the other providers to accurately determine the additional needs of veterans enrolled in Compact Act. Will educate the Veteran, their families, and the team of all the options available to them and will collaborate with the Veteran and family on the preferred option. The Social Worker will troubleshoot any issues that arise about eligibility, or other concerns, with veteran, providers, VISN, or others identified. Supportive counseling: The incumbent will provide Veterans and their caregivers with ongoing supportive counseling. The purpose of such counseling is to provide prevention and enhanced care services for those identified for Compact Act. Administrative responsibilities:The incumbent is responsible for developing and monitoring quality improvement measures related to Compact Act program metrics. This may include, but not limited to, ensuring providers have proper resolution of Compact Act. processes, proper completion of the required documentation, reviews for ongoing eligibility, and required outreach Compact Act. Flags. They formulate and implement an internal evaluation process through which progress toward goals and objectives can be objectively monitored. Additionally implements changes based on the results of evaluations and perform impact assessments of these changes. The incumbent may be responsible for ensuring compliance with Compact Act duties and collecting any relevant data requested by program leadership. The incumbent will enter all Veteran/family contacts in the electronic health record using appropriate formats and templates. This information will be entered in a complete, confidential, and professional manner to ensure information on the patient is shared with other VA staff. The incumbent will establish and maintain positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies. Participation in administrative committees associated with tracking and monitoring of Compact Act. eligible Veterans. Training/Development: The incumbent is responsible for designing and implementing new and ongoing employee training related to the Compact Act. This includes development of Compact Act. Workflow processes for all identified areas. They will ensure new providers have access to training materials, policies, directives, and an understanding of Compact Act. related duties. Qualifications

All requirements for this position must be met by the closing date of the announcement. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Must possess a Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Licensure: Must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Candidates must be proficient in written and spoken English to be appointed. Grade Determinations: Social Worker (Program Coordinator) GS-0185-12 (1) Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs (Note: a separate document with written demonstration of KSAs must be included with application, see required documents section): (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice.
