The Cincinnati, OH VA Medical Center is seeking an experienced Geriatrics and Extended Care (GEC) RN. The (GEC) staff nurse, provides nursing care to a wide variety of patients requiring short term care for rehabilitation, wound care, IV antibiotic therapy or end of life care and long-term care post hospitalization or in lieu of hospitalizations for maintenance care. Patient care frequently involves management of wound/skin problems and patients with high fall risk.

Registered Nurse - Geriatric and Extended Care
The Cincinnati VA is an affiliated teaching hospital, providing a full range of patient care services, with state-of-the -art technology, medical education and research. The Medical Center provides comprehensive whole health care through primary care, dentistry, specialty outpatient services, prevention and well-being services, and tertiary care in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, and neurology. The Community Living Center (CLC) is located adjacent to the Cincinnati VAMC that provides hospice, palliative care, short-term rehab, and long-term care.
The staff nurse (GEC) demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice in providing care to patients with basic or complex needs. This nurse has fundamental experience with the situations in which they are ultimately expected to perform. The focus in continued individual growth and development and demonstration self-direction with periodic oversight. The advanced beginner demonstrates fundamental leadership skills, shares knowledge, and develops others.
Registered Nurses are responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. He/she administers medications and procedures per established policies and guidelines. The staff nurse considers all characteristics of the individual, including sex and sex role identifications, age and life stage, state of health, race and culture, socio-economics status, values, and previous experience with similar events, in order to assist each patient to identify and meet basic health needs and improve his/her health status to the individual's maximum potential.
The staff nurse functioning as a preceptor assumes accountability and responsibility for the clinical development of assigned orientees. the staff nurse functioning as a charge nurse assumes accountability and responsibility for the care delivery provided to a unit of patients, by the assigned staff, for an assigned tour of duty.
Major Duties include but are not limited to:
Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care
Assesses, plans, implements, and evaluates care based on age specific components
Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
Considers all characteristics of the individual, including age, and life stages, state of health, race and culture, values and previous experiences.
Administers medications and procedures per established policies and guidelines.
Influences care outcomes by collaborating with members of the interdisciplinary team.
Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs

Qualifications
Basic Requirements:
Citizenship: Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)
English Language Proficiency: RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Graduation from School of Nursing:
Graduate of a school of professional nursing approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant.

4. License:
Condition of Employment. A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA.
Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I, Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I, Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I, Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree.
Physical Demands:
Per VA Directive and Handbook 5019 - a physical examination will be conducted based on the physical requirements of the position. A physical examination prior to placement is required. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
BLS/ACLS Requirements: Current certification in BLS is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high-risk areas (SICU/ICU/CCU/MICU/PACU/ED/and all procedure suites using moderate sedation, etc.). If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification prior to being boarded for appointment with VA. If selected, obtain such certification to avoid any delays in the hiring process.
