This position is responsible for the delivery of Whole Health Coaching and skill development in the Women's Clinic at the Dayton VA Medical Center. The position is responsible for provision of care through coaching, promotion, and education of Whole Health to Veterans and staff. The position is integral to supporting Whole Health coaches and Veterans in navigating the Whole Health journey, specifically through the Pathway, Wellbeing and Clinical Care elements of Whole Health care.

Duties

The position works with individuals and groups throughout the facility in a client-centered process to facilitate and empower the client to develop and achieve self-determined goals related to health. Coaches support clients in mobilizing internal strengths and external resources and develops self-management strategies for making sustainable, healthy lifestyle behavior changes and more effectively managing chronic disease. While health and wellness coaches do not diagnose conditions, prescribe treatments, or provide psychotherapeutic interventions, they provide expert guidance and may offer resources from nationally recognized authorities. Additional duties and responsibilities includes but not limited to the following: Contacts eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings. Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate. Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments. Guides Veterans in the exploration of core elements of personal health planning through individual and group based programs. Develops treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease. Reviews current health behaviors and develops customized treatment plans with Veterans. Coordinates care with Veterans' treatment providers, both primary and specialty care, to ensure identified goals are consistent with best practices for treatment and management of chronic illness. Coordinates with others on the Veteran's health care team to ensure optimal patient care. Uses VISTA, CPRS, Microsoft office software, Intranet and Internet, and other applicable software to track and support patient progress. Communicate proactively and effectively with Veterans, peers and supervisors. Develops and conducts onsite educational series for enrolled Veterans, family members, and VA employees to explain health coaching, including its uses and benefits. Participates in health fairs in the community. These activities can be planned in coordination with local Health Promotion and Disease Prevention, Veterans Health Education staff, patient-centered care coordinators, and other staff responsible for health-related education for Veterans Group coaching/educating on health coaching and training the facility. Provides eligible Veterans and family members with information regarding lifestyle management that are in line with the personalized health plan developed between the health care team and the Veteran. Develops information for Veterans about the Whole Health Group and Individual Programs. Conducts outreach activities to inform Veterans and family members of available programs. Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 09/26/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05, and for a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Basic Requirements: Social Science (GS-101) Occupational Requirements - The basic requirements for this occupation can be met by possessing one of the following: Education - 4 year degree in behavioral or social science or related discipline. (TRANSCRIPT REQUIRED) OR Experience - 4 years of appropriate experience that demonstrates acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. OR Experience/Education Combination - a combination of education and experience as described above. (TRANSCRIPT REQUIRED) Grade Requirements: In addition to the basic requirements, you must also meet the individual grade requirements to qualify for this position. You may qualify based on your experience and/or education as described below: Social Science Program Coordinator GS-7: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Coordinating and monitoring the functions of administrative services within an office; Managing budget activities; assisting with contracting functions and managing procurement; Incumbent must be familiar with information management systems, delegations of authority, management control systems, documentation procedures and control, and management directives at this facility. Making space utilization recommendations, etc. Education: One (1) year of graduate-level education or superior academic that provided you with the knowledge, skills, and abilities necessary to perform the duties of this position. (TRANSCRIPT REQUIRED) OR Specialized Experience/Education Combination: a combination of graduate education and specialized experience as described above. (TRANSCRIPT REQUIRED) Social Science Program Coordinator GS-9: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Comprehensive knowledge of whole health quality improvement measures; Knowledge of appropriate policies, directives, operating procedures, information flow, and automated practices in government agencies; Skill in retrieving tracking, and evaluating health coaching related information to find trends; Operational knowledge of eligibility, enrollment, income assessment, and insurance regulations; Knowledge of inpatient and outpatient programs, operations, admissions, scheduling, sharing agreements and travel; Operational knowledge of laws and regulations relating to the protection of private medical information; Knowledge of health coaching structure, processes, skills, and techniques' Knowledge of ethical code and rules of conduct for health coaches; Knowledge of information security principles and methods, etc. Education: Two (2) years of progressively higher level graduate education leading to a master's degree or equivalent graduate degree degree that demonstrates the knowledge, skills, and abilities necessary to do the work of this position. Physical Requirements: Regular and recurring work is mostly sedentary involving sitting at a desk, although the position requires the ability to move from one geographic location of the facility to another. Some physical effort such as standing, walking, bending or prolonged sitting may be required. The work places no special physical demands on the employee. Work is performed in an office setting with occasional visits to areas within the service as well as throughout the Medical Center. Work area is adequately lighted, heated and air conditioned.
