The Registered Nurse (RN) Lead CC&ICM Program Coordinator is administratively responsible for the clinical programming and daily operations of the facility CC&ICM program. Develops and implements policies and procedures specific to the facility and ensures compliance with local facility, VISN, Office of Nursing Service (ONS) and VA Central Office directives and handbooks, as well as the Joint Commission, and other accrediting bodies and regulatory standards as needed.

The incumbent provides structure and standards to support inter-professional collaboration and optimal utilization of healthcare resources. The incumbent assures that the physical, mental health and psychosocial needs are addressed to enhance Veterans' wellness, level of functioning, and quality of life with the goal of providing safe, efficient, and cost-effective interventions integrated throughout the continuum of healthcare. The incumbent utilizes a framework that provides standardized definitions and tools designed to integrate care horizontally and vertically to include but not limited to: bridging medical care and mental health care; transitioning inpatient care and outpatient care; and integrating community care and VA care thereby creating a seamless continuum experience for Veterans. The incumbent will provide support at the unit, service, and organizational levels. The incumbent will serve as a representative on medical center and service committees. The incumbent has expertise in coordinated care and serves as the integration liaison in a partnership between Executive Leadership and various programs and case managers/care coordinators. The incumbent co-manages the daily operations of the program and assists with ensuring facility-wide compliance with VHA Directive 1 1 10.04, Integrated Case Management Standards of Practice. The incumbent assists with the development of policies and procedures, contributes to the community of practice across the Veterans Health Administration through use of a data-driven quality improvement approach to case management practice with established goals and outcomes to evaluate and document case management effectiveness. The incumbent identifies and monitors case management metrics in collaboration with health informatics and prepares program and performance metric reports for VA medical facility leadership on a quarterly basis. Practice as an RN means this full scope of nursing, with or without compensation, that incorporates caring for all patients in all settings, through nursing standards of practice and professional performance for coordination of care, and may include, but is not limited to, all of the following: Collecting pertinent data and information relative to the patient's health or the situation on an ongoing basis through the comprehensive nursing assessment. Analyzing comprehensive nursing assessment data to determine actual or potential diagnoses, problems, and issues. Identifying expected outcomes for a plan individualized to the patient or the situation that prescribes strategies to attain expected, measurable outcomes. Implementing the identified plan, coordinating care delivery, employing strategies to promote healthy and safe environments, and administering or delegating medication administration according to Section 50-75 of the Act. Evaluating patient progress toward attainment of goals and outcomes. Delegating nursing interventions to implement the plan of care Providing health education and counseling. Advocating for the patient. Practicing ethically according to the American Nurses Association Code of Ethics Practicing in a manner that recognizes cultural diversity. Communicating effectively in all areas of practice. Collaborating with patients and other key stakeholders in the conduct of nursing practice. Participating in continuous professional development. Teaching the theory and practice of nursing to student nurses. Leading within the professional practice setting and the profession. Contributing to quality nursing practice. Integrating evidence and research findings into practice. Utilizing appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe and effective. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: BSN Two years of case management Certification in Case Management Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
