Duties and Responsibilities include, but are not limited to the following:
Manage medical care for a panel of Veterans enrolled in Primary Care
Timely entry and use of the electronic medical record system for documentation, workload capture, alert management, and for review of Veterans' current and future medical care needs
Serve as the leader of the Patient Aligned Care Team (PACT)
Cross-cover for other medical providers as needed throughout the health care system
Work in collaboration with PACT members to ensure the overall safety and quality of each Veteran's care
Responsible for all performance metrics related to patient quality of care
Responsible for positive customer service in all aspects of care
Participate in quality improvement, quality assurance, continuous survey readiness, and committee/work groups in collaboration with others
Maintain licensures and certifications requisite for credentials and privileges at the OKC VAHCS
Meet ethical and professional standards as written in the Medical Staff Bylaws and ensure that work relations are in compliance with facility standards and expectations as written and communicated
Driving and travel for Primary Care patient care coverage purposes will be required for this position to clinics throughout the health care system. Driving and travel may be up to 75% of the time.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected: i.e. Family Nurse Practitioner Certification (FNP-BC); Adult-Gerontology Primary Care Nurse Practitioner Certification (AGPCNP-BC)
Preferred Experience: Experience in Primary Care Clinical Setting - Family Nurse Practitioner or Adult Geriatric Nurse Practitioner
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level III - Master's degree in nursing (MSN) and no additional nursing practice/experience.
Nurse II - Master's degree in nursing and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Physical Requirements: The position involves considerable standing, walking, stooping, bending, reaching, lifting, and turning. It requires restraining confused patients, repositioning patients, assisting with transferring patients to Wheelchairs/stretchers/cars, and transporting them to other departments or facilities. Mechanical devices available and are required to be used for heavy or difficult to lift patients. The work may involve pushing or lifting weight in excess of 50 pounds and going up and down flights of stairs. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/ invasive procedures. This position requires visual acuity, good hearing, distinctive speech and manual dexterity. The
position requires the nurse to work for periods of time with arms above shoulder level with neck in a fully flexed or extended position and the ability to physically control.
