The Clinical Pharmacy specialist must be a licensed pharmacist preferably, with an accredited residency, or equivalent experience demonstrating competency. The Anticoagulation clinical pharmacy specialist works within the outpatient anticoagulation clinic practice setting to promote the optimal use of medications in view of patient disease state(s) and clinical response. This position's workload will consist of 50% anticoagulation work and 50% academic detailing.

Participate in daily multi-disciplinary team rounds or PACT huddles (when applicable) and provides drug information, observations on patient response to therapy, and appropriate recommendations regarding treatment alternatives or additional interventions to maximize patient care outcomes taking into consideration choice of therapy, safety, efficacy, and pharmacoeconomic. Interviews and evaluates patients upon admission to the hospital and daily during hospitalization for appropriateness of pharmacologic therapy and makes a pharmaceutical care plan for those patients who need pharmaceutical care, and will document the pharmaceutical care plan in the pharmacy initial admission progress note, i.e., non-VA meds, co-managed care meds, etc. Provides consultative services to all professional staff regarding drug therapy. Initiating medication orders as per approved protocol or provider consultation. Supports the Allergy / Adverse Drug Reaction Program. Clinical Pharmacy Specialists provide the above clinical pharmacy services, have an approved scope of practice that supports medication prescribing and/or highly complex clinical pharmacy services in support of direct patient care, work independently as supported by their scope of practice, and/or are responsible for the oversight, coordination and provision of a comprehensive pharmacy service-program. For all clinical pharmacists: Based upon the needs of Pharmacy Service enters prescriptions, counsels patients, enters non-formulary approvals into the VISTA tracking program and where required, dispenses medications. Participates in the development and implantation of formal and informal drug education / clinical consultation to medical, surgical, nursing, and pharmacy staff in the form of in-services and continuing education programs. Interfaces with other health-care providers and administrators to ensure compliance with established pharmacy and medical center policies and procedures. Works with other health care providers to develop programs for improvement of drug use. Develops and conducts drug usage evaluation in areas of needs from a cost and patient outcome perspective. Documents cost reduction / cost avoidance activities on a periodic basis. Supports the hospital formulary and ensures its appropriate utilization by health care providers. Consistently responds to patients, families, visitors, and co-workers requests for assistance promptly and in a friendly and cooperative manner. And deficiencies in this area are limited and minor and have no significant adverse impact on either organizational or individual productivity or efficiency. Listens to all customer / patient feedback, positive and negative, acts to resolve complaints within his or her control, and reports feedback to management (or supervisor) in a timely manner. Qualifications

Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an accreditation Council for Pharmacy Education accredited College or School of Pharmacy with a baccalaureate degree in pharmacy and/or a Doctor of Pharmacy degree. Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, an license(s)revoked, suspended, denied, restricted, limited or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Physical Requirements: See VA Directive and Handbook 5019. English Language Proficiency: Pharmacists must be proficient in spoken and written English. Grade Determinations: GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities: Ability to communicate orally and in writing to persuade and influence clinical and management decisions Expert understanding of regulatory and quality standards for their program area Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: PGY1, Experience in anticoagulation or academic detailing References: VA Handbook 5005, Part II, Appendix G15. The full performance level of this vacancy is GS13. Physical Requirements: The work involves prolonged periods of standing with some walking of generally short distances. There may be occasional lifting of weights up to 30 lbs. There may be mental fatigue due to repetitive screening and interpretation of medication orders. Light carrying (under 15 lbs.), Reaching above shoulder; use of fingers, both hands required; walking (up to 4 hrs.), standing (up to 4 hrs.),both legs required, both eyes required, ability to distinguish basic colors; ability to distinguish shades of colors, hearing (aid permitted).
