The PRRC is an intensive outpatient specialty mental health transitional learning center designed to support recovery and integration into meaningful self-determined community roles for Veterans challenged with serious mental illness and severe functional impairment. The PRRC provides services to Veterans within the CAVHS catchment area which includes Community Based Outpatients Clinics (CBOCs). PRRC programming is multimodal (e.g., curriculum-based classes, counseling, and in-vivo exercises) and is developed and implemented to capitalize on each Veteran's skills, strengths, and talents. Additionally, programming focuses on building skills and accessing resources to define and realize one's self-chosen roles and goals in all domains of wellness. PRRC services are part of the mental health continuum of care and are delivered by an integrated, multidisciplinary team with a focus on a high intensity of care, crisis prevention and management, flexibility, and community-oriented treatment settings. Community inclusion with reference to PRRC is cultivated through partnership, education, and advocacy among Veterans, PRRC staff, VA medical center, and community members. The Social Worker Program Coordinator oversees the day-to-day operations of the PRRC, coordinates mental health, physical or psychosocial services and ensures that program staff adhere to VHA Directive 1163, Psychosocial Rehabilitation and Recovery Services. The Social Worker Program Coordinator is responsible for managing to day to day operations of the PRRC. Program responsibilities include developing, planning and monitoring programming to ensure diverse treatment options are available for Veterans traditionally identified as living with Severe Mental Illness (SMI). Program coordination duties include ensuring all scheduled programming occurs and is high-quality, ensuring compliance with NEPEC program monitoring protocols, and ensuring compliance with national guidance and CARF accreditation requirements. Additionally, the PRRC Coordinator is assigned to work with patients who often have highly complex physical, psychosocial and mental health problems, such schizophrenia, schizoaffective disorder, bipolar with psychosis and other psychotic disorders. The Program Coordinator must frequently make clinical decisions without immediate access to social work supervision in the community while facilitating groups and other community-based activities. The incumbent utilizes a range of interventions to include evidence-based treatment modalities for Veterans with serious mental illness, which includes but not limited to Whole Health Coaching, Motivational Interviewing, Illness Management Recovery, Cognitive Behavioral Therapy, Acceptance Commitment Therapy for Psychosis and other evidence-based practices. The incumbent is responsible for linking Veterans to other VA programs and community resources, while serving as a bridge to such resources. The incumbent facilitates interdisciplinary team staffing regarding admissions, consults, referrals, treatment planning, progress evaluations, and transition planning. The Social Worker Program Coordinator works with CAVHS inpatient teams to assure continuity of care, which could include attending interdisciplinary staff meetings, providing consultation regarding clinical needs, treatment planning and case management services. The incumbent provides mental health in-services to the interdisciplinary treatment team and other mental health programs, to include education on evidence-based treatment practices, psychotic disorders, current research, etc. Work Schedule: Monday through Friday, 8:00 am - 4:30 pm Telework: 1 - 2 days per pay period Virtual: This is not a virtual position. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: SOCIAL WORKER (Program Coordinator), GS-12 Experience/Education: In addition to meeting the basic requirements, applicants must also meet the grade requirements for the particular grade he/she wishes to be considered. For GS-12, applicants must have One year of experience equivalent to the GS-11 grade level. Examples of creditable experience include but not limited to assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence, advocate with appropriate VA and community service providers and agencies, independently assesses the psychosocial functioning and needs of Veterans and their family members, collaborate with the Veteran, family/caregiver, and interdisciplinary treatment team to facilitate the delivery of health care services, conducts assessment and provides specific interventions, incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses, independently formulate, provide a range of interventions and treatment modalities and implement a treatment plan. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Resume must include hours per week for work experience credit. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate all of the following KSAs: Social Worker (Program Coordinator), GS-12 (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. (KSAs must be clearly reflected in resume under description of duties in order to be found qualified for this position/grade level). Preferred Experience: 3 years working with individuals with serious mental illness At least one year experience in program development or process improvement of programming Performs mental health evaluations for a diverse population, including geriatric and post-deployment combat Veterans, who have a serious mental illness, such as schizophrenia, schizoaffective, and bipolar disorders and who may also have psychosocial issues such as homelessness, unemployment, or a poor family support network. Process a full range of therapeutic interventions including diagnostic assessments, suicide risk assessments, crisis intervention, evidence-based psychotherapy, and overall care of patients with mental health disorders. Modalities for patient care include face-to-face, video teleconference, and telephone calls as clinically appropriate. Provider must be able to incorporate alternative encounter modalities into clinical care. Ensures practice of mental healthcare is consistent with TJC, CARF, HIPPA. VA policies and procedures, clinical practice guidelines, national statutes and regulations, medical records documentation, and maintain mandatory continuing education. Participates effectively in team meetings and collaborates with interdisciplinary team members to enhance coordination of comprehensive patient care. Orders consultations appropriately.
