The PACT High Risk Case Manager/Social Work Coordinator acts as an integral part of the primary care team and is responsible for ensuring that the Patient Aligned Care Team maintains a central role in coordinating and management the care of these vulnerable patients. This position is located in Southern Colorado, tasked with primary coverage of high-risk patients in the Pueblo, Colorado catchment area.

Duties

The High Risk Case Manager/Social Work Coordinator is the point person for complex patients, making sure that services are coordinated and communicated between all team members. Incumbent will work with an interdisciplinary team including behavioral health, case management, and primary care to establish goals and a plan of care for each complex patient. This position will have a caseload that encompasses Pueblo and greater Southern Colorado and will be expected to provide on-site care at the Pueblo CBOC. This position will provide coverage as needed across all PACTs, on-site and virtually, within VA Eastern Colorado Health Care System, as necessary. The position requires occasional driving and the ability to drive between southern Colorado sites. The incumbent must hold a valid driver's license and be able to operative government issues vehicle. The incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans and their families/significant others. Social Work responsibilities include the assessment of psychosocial problems that cause distress, often impacting the health condition and creating barriers to care. Must have a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans in the Primary Care Clinic and/or their families. The incumbent is able to independently work with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and psychosocial problems. Effective social work in the Patient Centered Medical Home context often occurs over a series of several visits. Must independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. The incumbent must be able to utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable. Must provide consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs, and advance directives. This includes the incumbent's knowledge of the process for accessing and/or coordinating community-based services, including information and referral for additional services from other VA programs, other government programs, and community programs. Is responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), Veterans Integrated Service Network (VISN) 19, and VA Eastern Colorado Healthcare System.. Must be available to provide consultation to colleagues and students regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning and compliance with treatment, role modeling effective social work practice skills. Demonstrates the ability to teach and mentor staff and students in the special area of practice and to provide supervision for licensure or specialty certifications. Serves on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of Social Work Services. The GS-12 PACT High Risk Case Manager/High Risk Social Work Coordinator performs duties independently, reporting to the Section Chief, Primary Care, Care Management and Social Work and to the Chief, Social Work Service. Due to the location of clinical needs of the clinic this position service, the social worker is required to be a Licensed Independent Provider. Incumbent must meet the needs of customers while supporting VA Missions; consistently communicate and treat customers (Veterans, their representatives, visitors, and all VA Staff) in a courteous, tactful, and respectful manner; provide customer with consistent information according to established policies and procedures; and handle conflict and problems in dealing with customers constructively and appropriately. Incumbent willingly accepts additional and special assignments that are necessary to meet program goals or customer needs. Basic Requirements: Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part) English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to the social worker position GS-0185 series in the VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: Social Worker (Program Coordinator) GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignments: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Has a minimum of one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. The incumbent must be licensed or certified by a state at the advanced practice level. The incumbent adheres to the NASW Code of Ethics. The full performance level of this vacancy is GS-12. Physical Requirements: The work is sedentary. Typically, the employee sits comfortably while working. However, there may be some light exertion required when walking, standing, bending, driving, climbing stairs, carrying of light items such as papers, books, small objects, or driving an automobile. No special physical demands are required to perform the work. Ability to complete a Basic Life Support (BLS) course is a position requirement.
