The Registered Nurse Care Coordination and Consult Specialist is assigned to the TVHS Clinical Operations - Referral Coordination Section. The Nurse functions directly to support the activities of the Clinical Practice Management and assigned tasks related to care coordination and consults.

Duties

The Registered Nurse Consult Specialist is assigned to the TVHS Clinical Operations Service's Referral Coordination Section. The Nurse functions directly to support the activities of the service and assigned tasks related to care coordination and consults. The Nurse is responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran's care. The foundational elements will serve to support delivery of high quality, timely care to the Veteran receiving services in the community. The Registered Nurse is the lead of the integrated care team established by the VA Community Care Operating Model. He/she is responsible for providing clinical leadership and expertise on consults, community care processes and regulations. The Registered Nurse will serve as a liaison to both internal and external providers and will be responsible for managing the care of the Veteran and the seamless transition to and from the community. He/she possesses the knowledge related to VHA guidelines, utilization review criteria, current evidence-based standards of care, compliance guidelines and local policy related to Delegation of Authority as established by the facility Chief of Staff. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. The nurse's primary commitment is to the patient, whether an individual, family, group, or community. The Nurse demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The nurse is a PROFICIENT nurse, "who understand a situation as a whole, because they perceive its meaning in terms of long-term goals. The proficient nurse learns from experience what typical events to expect in each situation and how plans need to be modified in response to these events. The proficient nurse can now recognize when the expected normal picture does not materialize; decision-making is less labored. The proficient nurse considers fewer options and hones in on an accurate region of the problem" (Benner), identifying emerging issues on the unit that need to be addressed. The Nurse practice affects unit, team, or work group. The Nurse demonstrates the ability to identify an issue, analyze it, and develop and implement interventions. The Nurse looks for opportunities to improve practice or functions at the unit level for a group of patients, using group process to accomplish this, for example, organizing, leading, or facilitating interdisciplinary team meeting. Care will be provided in the least restrictive, most cost-effective manner through use of the clinical practice guidelines and clinical pathways. Evidence of health promotion and chronic disease indices will be monitored through the clinical practice guidelines and performance measures. The eight core competencies of the High Performance Development Model (HPDM) and the Veterans Administration Core Values of ICARE incorporated into the dimensions of practice that support their application. Pay scale: NY 48 Work Schedule: Monday-Friday (Based on the needs of the service) Telework: Position is telework eligible at manager's discretion Virtual: This is not a virtual position. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Minimum of two years Ambulatory Care experience. Case Management/Consult Management experience. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: You must be able to perform work that requires regular and recurrent bending, lifting 30 pounds or more, stooping, climbing, twisting and/or bending, standing and/or walking almost constantly during the course of the day, reaching above the shoulder, stretching, and similar activities supporting various weights; walking and standing for 8 hours; frequent pushing medication carts, wheelchairs, litters, beds; use of fingers, both hands and both legs; emotional stability (you must be able to care for Veterans with various medical and psychiatric illnesses who at times may exhibit unpredictable behavior and may have the potential to be physically or verbally assaultive); near vision correctable at 13" to 16" / far vision correctable in one eye to 20/50 and to 20/100 in the other / both eyes required with ability to distinguish basic colors; hearing aid permitted in order to sufficiently hear various alarms and respond immediately
