GENERAL DESCRIPTION OF ASSIGNED DUTIES.
The Caregiver Support Program Coordinator for the Program of Comprehensive Assistance to Family Caregivers (PCAFC) is a Social Work Program Coordinator whose primary responsibilities are to provide clinical evidence-based services and interventions; program development; caregiver, Veteran, and staff education on caregiver issues; community outreach; resource development; continuous quality improvement activities and evaluation/consultation related to the PCAFC. The coordinator will function in support of caregivers, Veterans, and family members throughout the Medical Center and in the community and will support the development and implementation of General Caregiver services. This includes working with the facility CSP Program Manager to provide program support and oversight of the PCAFC under the umbrella of Caregiver Support Program services. Tasks include but are not limited to, completing Veteran and caregiver assessments, collaboration with team members and other VA providers, in-home visits to complete Wellness Contacts, tracking consults and workload, covering for other team members, and conducting frequent needs assessments to assist with implementing education, wellness, and support groups, among other duties. In addition, the coordinator must be a skilled communicator capable of developing effective partnerships with local, state, and federal agencies as well as non-governmental organizations.
The coordinator is administratively responsible for the clinical programming and daily operations of the Program of Comprehensive Assistance to Family Caregivers and may serve as back-up to the facility CSP Program Manager. Assists with the development of policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. The coordinator may be responsible for the program's budget and various control points assigned to the program within the assigned areas.
The coordinator manages and controls the use of data related to these programs regarding day to day functioning and program evaluation. Plans, organizes, and directs administrative, operational, caregiver, Veteran, and personnel activities for the program. The GS-12 Caregiver Support Coordinator has experience that demonstrates the advanced practice skill level in social work. At least two years of experience should be in an area of specialized social work practice. The coordinator must be independently licensed by a state to practice at the advanced practice level. The coordinator will function in support of caregivers, Veterans, and family members throughout the medical center and in community settings. The GS-12 Caregiver Support Coordinator is practicing at the advanced level and has specialized knowledge of practice that can be differentiated from the independent practice social worker by the ability to expand clinical knowledge in the profession, administer a facility PCAFC, provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification. 2 The Caregiver Support Coordinator position requires advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state, and private sector resources. As an advanced practice level clinical social worker, the coordinator is assigned to work independently with multiple VA healthcare services and with community agencies, leaders, and organizations where he/she has no access to direct social work clinical supervision. The coordinator is responsible for developing and establishing linkages and collaborates on VA caregiver support efforts with community healthcare and social service agencies. The coordinator is administratively responsible and accountable for the clinical and program development and effectiveness of the facility PCAFC. The coordinator will adhere to the NASW Code of Ethics, all local social work policies, and maintain an unrestricted, unsuspended license to practice social work.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
SOCIAL WORKER (PROGRAM COORDINATOR), GS-12
Experience/Education: The candidate must have at one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Resume must include hours per week for work experience credit.
Licensure/Certification: Must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.

Preferred Experience:
- LCSW with 2 years of medical social work experience.
- Adaptability and flexibility, planning and organizational skills, self motivated, interpersonal skills, ability to work in a fast-paced environment, teamwork skills, excellent computer skills, and excellent evaluation and assessment skills.
