The Associate Chief Pharmacy Clinical is an experienced clinical pharmacy specialist who holds a Doctor of Pharmacy degree or equivalent qualification and experience per current VHA Directive. This position is responsible for managing pharmacy clinical activities at Spokane VA. This includes the provision of leadership to the pharmacy clinical program and work to expand pharmacy's clinical role at Spokane VAMC.
Duties
The Associate Chief Pharmacy Clinical is responsible for managing the Anticoagulation Program, Pharmacy Collaborative Consult Clinic, Pharmacy Metabolic Clinic, Pharmacy Renewal Clinic, Traveling Veteran Program, Methadone Order Review Consults, and clinical pharmacy affiliations with Universities and pharmacy residency programs. The incumbent also participates in Clinical System Redesign Projects within and across service lines. Duties will include:
Represents clinical pharmacy on local P&T Committee, VISN 20 P&T Committee and VISN 20 Pharmacy Advisory Committee.
Manages Adverse Drug Events and Medication Errors Reporting Program (VA ADERS) for Primary Care and reports adverse drug events to the PT&N Committee. Serves as champion to translate adverse events and reporting trends, and provides suggestions for improvement.
He/she will provide leadership in building and managing affiliations with Colleges of Pharmacy, development of Pharmacy Residency Program and provides mentoring and training to WSU and ISU Ambulatory Care pharmacy students.
Responsible for MUE development and presentation and assists pharmacy staff and students in this endeavor.
The incumbent is responsible for Spokane Pharmacy Clinical Programs. He/She provides direct patient care in all facility pharmacy clinics as needed, while mentoring and educating the clinical pharmacy staff assigned to the pharmacy clinics.
This position is responsible for managing expanded Scope of Practice/Credentialing & Privileging for all Clinical Pharmacists and is responsible for all associated quality monitors and updates as needed.
The incumbent evaluates results of non-formulary medication consults that are approved or denied by clinical pharmacy for Spokane VAMC. Any trends noted are reported to Chief, Pharmacy Service and Spokane VA PT&N Committee as needed. Provides monthly updates on formulary issues and criteria to staff responsible for reviewing non-formulary and restricted requests.
Actively participates at VISN 20 P&T committee meetings and helps develop quality improvement strategic objectives at a VISN level.
This position is responsible for participation in various activities to enforce and enhance the role of pharmacy as an active member of the health care team.
The incumbent provides resources and support to staff pharmacists/pharmacy specialists in order to provide the highest quality clinical pharmacy services.
This position is responsible for managing protected Pharmacy Peer Review as needed by Quality Management.
The incumbent is responsible for monitoring, developing and evaluating clinical activities provided by pharmacy staff with an emphasis on pharmaceutical care concepts.
Responsible for managing and developing all complex clinical pharmacy programs, including the development, organization, maintenance, expansion, and staffing of pharmacy clinics.
This position is responsible for the Anticoagulation Therapy Clinic Program, both inpatient and outpatient. He/She is responsible for designing new guidelines for management of anticoagulation to increase efficient and seamless transition from inpatient to outpatient. This includes developing protocols and supervising education and training. Maintains certification for anticoagulation management and provides backup coverage if required.
He/She is responsible for management of all aspects of anticoagulation therapy to ensure patient safety, staff education and assessment of staff competency across the medical center.
Develops and updates policies and procedures for all clinical pharmacy programs.
The incumbent designs and manages clinical outcomes monitors aimed at improving values with clinical processes. Including training of staff and Clinical Pharmacy Specialists to expand capabilities and improve effectiveness in monitoring drug therapy and providing other clinical services.
Provides leadership to develop best clinical practices and present successes at VHA regional and national meetings.
The incumbent is responsible for managing and supervising pharmacy activities as needed and directed by the Chief of Pharmacy.
The incumbent provides project management support to the Chief of Pharmacy.
Prepares and presents reports to the Clinical Executive Council annually and serves as alternate for Clinical Executive Council with Chief, Pharmacy Service.
The incumbent is the first line supervisor for professional staff on the approved organizational chart. The incumbent will supervise and coordinate students and residents assigned.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Education:
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-14, Associate Chief
Assignment: Responsible for coordinating multiple programs in pharmacy service in a medical facility. Serves as the Acting Service Chief in the absence of the Service Chief. Develops and maintains a system of internal reviews that ensure service programs operate at a satisfactory level of performance and in compliance with regulatory and accrediting bodies. Responsible for pharmacy utilization of resources and budget. Makes selections, assigns personnel, and serves as a mentor to help employees develop their full potential.
Experience. In addition to the GS-13 requirements, must have 1 year of experience equivalent to the next lower grade level.
