The primary purpose of the position is to work collaboratively with the PACT Teams in facilitating care coordination through consultative, procedural, laboratory and radiological records retrieval from community providers. Record retrieval will be achieved through use of available technology and platforms and may include telephonic calls, facsimile, and/or internet-based portals.

Duties

MAJOR DUTIES: Check documents from portals (i.e., CPRS and JLV) and document findings. Contact community provider after 14 days of appointment and request for notes. Correctly identify records associated with specialty consults and retrieve notes as needed prior to PACT appointment. Ensure that all electronic correspondence is sent to correct destination, complying with HIPPA and Privacy Act Laws Identify and index documents after verifying correct vendor and validity dates for the consult as needed. Answers telephone calls to clinic and directs telephone calls/messages to appropriate service/teams. Work Schedule: Monday-Friday 7:30am-4:00pm Position Description Title/PD#: Health Technician/PD03504A Physical Requirements: The work requires long periods of walking, bending, standing, and sitting, pushing and/or pulling, and positioning patients. The work required also includes assisting in lifting and supporting Veterans and the ability to carry up to 20 pounds. It also includes computer, keyboard, and telephone work.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement. Individual Occupational Requirements Specialized Experience (for positions at GS-4 and above): Positions in this series range widely in type and include support duties to medical or health personnel such as audiologists, speech pathologists, medical officers, and optometrists. Therefore, technician experience is experience that required application of the knowledge, methods, and techniques of the position to be filled. or Education and Training For GS-5: Successful completion of a full 4-year course of study leading to a bachelor's degree, with major study or at least 24 semester hours in subjects directly related to the position. GS-05 grade level: One year of specialized experience (equivalent to the GS-XX grade level in the federal service); experience that equipped the applicant with the particular knowledge, skills and abilities (KSA's) and other characteristics to perform successfully the duties of the position, and that is typically in or related to the work of the position to be filled, in the normal line of progression for the occupation in the organization. Specialized experience includes: Knowledge and skill to perform a range of clinical and clerical support. Knowledge of the elements of a patient-driven holistic care plan for life and skill sufficient to provide appropriate verbal information and written facts. Knowledge of medical terminology and sufficient skill to apply to practice in order to assure the appropriate/safe use of equipment and supplies. Knowledge of principles of infection control and principles of safety OR Applicants may substitute education for the experience required for the GS-05 level. Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have 4-year course of study above high school leading to a bachelor's degree with courses related to the occupation, if required OR Applicants may also combine education and experience to qualify at the GS-05 level. To calculate, first identify the percentage of required education you have earned (when substituting, you cannot use the first 60 semester hours that you earned). Then identify the percentage of required experience you possess. Add the two percentages. The total percentage must equal at least 100 percent to qualify at the GS-05. For example, an applicant who has 9 semester hours of graduate level education (i.e. 50% of graduate education required) and 6 months of creditable specialized experience (i.e. 50% of the required experience) would equal 100% of the required experience for the position A transcript must be submitted with your application if you are basing all or part of your qualifications on education.
