The incumbent serves as a Licensed Practical Nurse (LPN) at the point of care for the Kansas City VA Medical Center, Primary Care Service's Community Based Outpatient Clinic located in Excelsior Springs, MO. The incumbent works under the direction of an RN and is part of the Patient Aligned Care Team (PACT) with a focus on patient centered care delivery. The LPNs function is to provide quality nursing care to a culturally diverse veteran population of varying complexity in a variety of settings.

Duties

Major duties include, but are not limited to: Observing, recording and reporting changes in the patient's condition. Assisting in the development and revision of the patient-driven holistic life care plan. Demonstrating active collaboration, communication and problem solving with Core Team (Patient, Primary Care Provider, RN Care Coordinator and Clerk) and Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). Participating as part of a high functioning team in meeting the PACT initiatives and outcomes. Communicating and interacting appropriately and courteously with internal and external customers. Maintaining confidentiality of patient/employee information electronically, verbally, and written. Working and functioning within a safe manner including infection control and operation of equipment. Integrating and applying pertinent aspects of the Medical Center performance improvement program into his/her professional practice. Utilizing nursing research, evidence based practice and literature in daily practice. Providing care appropriate to the bio-psychosocial needs of the population served. Understanding the changes associated with aging and principles of growth and development relevant to the adult and geriatric population. Assessing and interpreting data relating to the patient's current health status. Facilitating the delivery of care appropriate to the age specific needs of the adult and geriatric patient. Providing culturally competent care to a diverse patient population. Ensuring qualifications and required certifications are maintained, including Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Advanced Trauma Life Support (ATLS) certification. Scheduling clinic appointments using the VISTA Scheduling System in accordance with the business rules outlined in VHA Directive 2010-027, Outpatient Scheduling Processes and Procedures. Demonstrating knowledge and use of general business rules outlined in VHA Directive 2010-027, Outpatient Scheduling Processes and Procedures. Utilizing established business rules for Enrollment and Registration, Scheduling, Consult Management, Use of the VistA Electronic Wait List (EWL), Handling No-Shows, Patient Cancellations, and Clinic Cancellations as applicable to job. Utilizing insurance software, as instructed, to insure the capture of all insurance information during the check-in process. Work Schedule: Monday - Friday, any 8 hour shift between the hours of 7:00am - 5:00pm Telework: Ad-Hoc Virtual: This is not a virtual position. Qualifications

Basic Requirements: United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment OR individuals granted a license by a jurisdiction that does not require graduation from an approved school AND have at least one year of successful practice as an LPN/ LVN. Licensure. Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico), of the United States or the District of Columbia. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grade Determinations: In addition to the basic requirements above, candidates must meet the following grade requirements. GS-06 Experience: Candidates must have the completion of one (1) year experience at GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN. AND in addition to the experience above, candidates must demonstrate all of the following KSAs: Demonstrate technical proficiency in initiating, performing and completing assigned duties in providing care to patient populations at all complexity levels and care settings. Knowledge and ability to appropriately carry out assigned patient care based on the patient's conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant patient information. Ability to observe, identify and respond to the patient's need for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN recognized and considers emotional, cultural, spiritual, socio-economic and age-related factors. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD. Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and or MD, and initiate appropriate emergency interventions as directed. Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members and staff. Established constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills. Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery. Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members and staff. Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which includes preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during and following the procedure. Serves as a preceptor in orienting, training and educating less experienced LPNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures. Actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice. Grandfathering Provision. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Preferred Experience: At least one year outpatient nursing experience and BLS IV certification is preferred. Physical Requirements. See VA Directive and Handbook 5019.
