This position is located at the Behavioral Health Department at the Anadarko Indian Health Center in Anadarko, OK. This position works to increase the provision of behavioral health services that promote the efficient and effective use of a variety of health care resources to provide a seamless continuum of care. Temporary - 1 Year May be extended up to a maximum of four years.

Duties

Provides comprehensive case management for patients by coordinating behavioral health services for patient's needs. Identifies, assesses, and coordinates care for patients in mental health crisis utilizing evidenced based practices to implement care in the least restrictive environment best for the safety of the patient. Identify patient's problems, strengths, weaknesses, coping skills, and assistance needed. Update patient electronic medical records charts as needed; document all contacts and attempted contacts with patients in RPMS. Schedule and manage patient appointments in a computerized database, RPMS. Screens potential patients identified through the triage and screening for BH case management.

You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. Basic Requirements Degree: behavioral or social science; or related disciplines appropriate to the position. or Combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. or Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: GS-07 Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Provides guidance, arranges in-takes, coordination, and collaboration regarding Case Management/Discharge Planning. Refers and assists patients to appropriate Mental Health facilities and community resources. OR Completed 1 year (18 semester hours) of graduate-level education that is directly related to the work of the position or superior academic achievement. OR Combination of education and experience, in one or more of the behavioral or social sciences equivalent to a major in the field, that when combined fully meet the minimum qualifications for this position. MINIMUM QUALIFICATIONS: GS-09 Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Monitor and follows up on the implementation of the written service plan. In depth collaboration and knowledge to link clients with necessary community resources, medical providers, knowledge in assisting with obtaining access to needed medical, psychiatric, social, educational, etc. while maintaining contact throughout the treatment plan. OR Completed 2 years (36 semester hours) of graduate education leading to a master's degree or master's that is directly related to the work of the position. OR Combination of education and experience, in one or more of the behavioral or social sciences equivalent to a major in the field, that when combined fully meet the minimum qualifications for this position. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
